Premise Health is bringing high-quality, convenient healthcare services with no out-of-pocket cost to participants and eligible family members covered by the Pipe Trades Services MN Welfare Fund.
Here’s what you need to know.
The existing Pipe Trades Health & Wellness Center locations in White Bear Lake, Eagan, Maple Grove and Rochester will close from December 30 – January 24 to transition to Premise Health. Please plan to refill any prescriptions you have at the clinics ahead of time to ensure you have everything you need prior to the closures.
Virtual primary care is now available! While the physical clinics are closed you can access care, virtually. To learn more, visit members.premisehealth.com/virtual-primary-care.
You will have access to the following services that were available prior to the transition:
Primary Care
Pediatric care
ages 2+
Behavioral health
Chiropractic care
Provider dispensed
medications
Vision
Physical therapy
You now have access to this new service:
Enhanced Virtual Primary Care
This includes access to a dedicated provider by phone or video for all your primary care needs and an on-demand care team who is available 24/7, so you can get the care you need, anytime, anywhere – even when you’re traveling.
