How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

After turning onto White Bear Pkwy, take your fourth right-hand turn in the industrial park area. Continue straight back until you reach the end and turn left. The clinic entrance is at the end of the building on your left.

The Health & Wellness Centers are open:
- Monday and Wednesday 7 a.m. – 5 p.m.
- Tuesday and Thursday 8 a.m. – 6 p.m.
- Friday 7 a.m. – 11 a.m.
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Who is eligible to use the Pipe Trades Family Health & Wellness Centers?
Participants in the Pipe Trades Services MN Welfare Fund and their dependents (ages 2 and up), and retirees, including pre-Medicare retirees and Medicare-eligible retirees.
How do I schedule an on-demand virtual primary care visit?
If you have not done so already, first set up your account on the My Premise Health app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will then put you in touch with your provider to begin your visit.
Please note: virtual primary care cannot provide diagnostic treatment for Pipe Trades members on Medicare Advantage plans; only triage and guidance are available. This service is not available for Medicare/BCBS Advantage Plan members.
How much does it cost to be seen by a provider at Pipe Trades Family Health & Wellness Center?
Is my health information secure?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
