Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

 

The Pipe Trades Services MN Welfare Fund has hired Premise Health to bring its participants and their eligible dependents healthcare services and operate at the current Pipe Trades Family Health & Wellness Center locations.

You are considered a member because the Pipe Trades Services MN Welfare Fund has hired Premise Health to provide health and wellness services to you and your eligible family members.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

Pipe Trades Health & Wellness Centers are not staffed or managed by Pipe Trades Services MN Welfare Fund or any of its related entities or employees in any way.

While where you receive care is a personal choice, we hope you’ll consider trusting us with your overall wellbeing, much like the Pipe Trades Services MN Welfare Fund has trusted us to operate its wellness centers.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your health by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see your test results and more.

Eligibility and cost

Participants in the Pipe Trades Services MN Welfare Fund and their Dependents, and Retirees, including Pre-Medicare Retirees and Medicare-eligible Retirees.

There are no fees associated with any of the services utilized in the health centers.

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or union or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members. You can review a notice of privacy practices within Pipe Trades Health & Wellness Centers.

Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP.

Scheduling

Once you create a My Premise Health account, you can easily schedule a visit through the My Premise Health app or online at MyPremiseHealth.com. Simply log in, select “Schedule an Appointment” and choose your appointment type.

You can also call your wellness center or visit during regular center hours to make an appointment.
If you’re experiencing a medical emergency, please dial 911.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

Virtual Primary Care is a digital healthcare service through Premise Health. It offers management of your ongoing care needs as well as 24/7/365, on-demand access to care with a virtual primary care provider (PCP) from anywhere within the United States. Virtual primary care visits are available through the My Premise Health app or online at mypremisehealth.com by phone or video.

While Virtual Primary Care team members are not Pipe Trades Family Health & Wellness providers, the visit notes generated during your Virtual Primary Care visit can be viewed by your Pipe Trades Family Health & Wellness provider.

Yes.  The purpose of Virtual Primary Care is to provide you with on-demand access to care, on a 24/7/365 basis.  If you would like to see a provider right now, start by logging in to your My Premise Health account through the My Premise Health app or at mypremisehealth.com. On your dashboard, select “Get Care Now.” From there, simply follow the prompts to see the next available provider as soon as possible.

If you have not done so already, first set up your account on the My Premise Health app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will then put you in touch with your provider to begin your visit.

You can also call (888) 535-4980, and our 24/7/365 Virtual Primary Care team will walk you through the process.

Yes, much like Virtual Primary Care, your Pipe Trades Family Health & Wellness Center provider can do a virtual visit by phone or video. Visits can be scheduled through the My Premise Health app, online at mypremisehealth.com, or by calling the location you wish to be scheduled at. These appointments are only during normal health centers hours.

You must be physically located in the State of Minnesota to access a Pipe Trades Family Health & Wellness virtual visit.  If you are physically located in a different state, you can still use the 24/7/365 Virtual Primary Care service.

Virtual Primary Care visits are just like in-office visits. You will discuss your medical needs with your provider, and they will diagnose and prescribe medications when medically appropriate. Virtual Primary Care providers do not prescribe controlled substances, non-therapeutic drugs or other drugs that can be harmful due to their potential for abuse.

Virtual Primary Care was designed to treat your preventive, short-term and long-term health needs. If you are experiencing a medical emergency, please dial 911 to get the medical attention you need.

It depends on when prior treatment was received before or after January 24, 2023. 

Records Before January 24, 2023:  The 24/7/365 Virtual Primary Care service does not have any access to your records for treatment received at the Pipe Trades Family Health & Wellness Center prior to the January 24, 2023 transition to Premise Health.  

Records On Or After January 24, 2023:  All services received after the January 24, 2023 transition to Premise Health can be viewed by the 24/7/365 Virtual Primary Care team in EPIC, as they are integrated with the onsite health centers. In addition, although Virtual Primary Care team members are not Pipe Trades Family Health & Wellness providers, the visit notes generated during your 24/7/365 Virtual Primary Care visit can be viewed by your Pipe Trades Family Health & Wellness provider.

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

 

 

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
