Primary Care
We offer a comprehensive range of primary care services that focus on your long-term health and preventive care. Your primary care provider will get to know you and your family, so they are better equipped to prevent, treat and manage any health issues that may come up. The best part? You can schedule same-day, next-day and virtual appointments and message your provider on your secure patient portal, My Premise Health. Vision
Good eye health is vital to your overall wellness, which is why we offer a full range of vision care to you and your family. Our optometrist can meet all your eye-care needs with our state-of-the-art equipment all within Pipe Trades Family Health & Wellness Center. You can check another office visit off your list and get the care you need all under one roof.
Our vision services include comprehensive eye exams, lens fittings, color vision exams, foreign body removal, glaucoma care, occupational eye exams, visual field testing and the diagnosing, managing and treatment of ocular disease.
Provider Dispensing
Skip the extra trip to the pharmacy. You can get most or many of your medications at no cost directly from your provider during your visit. If medications are not available through the wellness centers, your provider can send a prescription to the pharmacy of your choice. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
Pipe Trades Family Health & Wellness Center carries close to 200 medications to cover your needs. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.
Physical Therapy
You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your wellbeing. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.
We offer a full range of services for you and your family, in-person or virtually, that include therapeutic exercise and conditioning, pre- and post-operation rehabilitation, ergonomics evaluations onsite or virtually, orthopedic rehabilitation, health management and awareness education.

Women's health
Women have unique healthcare needs. Your provider will spend time getting to know you, or the women in your life, to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. Along with general primary care services, we offer the following specifically for women’s health: breast cancer screenings, pregnancy testing, pelvic exams, contraception counseling, STD testing and counseling, cervical cancer screenings (Pap smears), counseling for interpersonal and domestic violence, and LGBTQ+ care.
Women’s health services are available for all Pipe Trades members and their eligible dependents.
Lab services
To support you and your family’s needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.
We can perform and process the following lab tests onsite: glucose or lipid testing, rapid strep testing, influenza testing, urine pregnancy testing, urine dip screenings, rapid mono testing and COVID-19 testing.

Behavioral health
At Pipe Trades Family Health & Wellness Center we believe that your mental and emotional wellbeing are just as important as your physical health. With our behavioral health services, you and your eligible dependents can get the support you need. Your licensed counselors will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently reach them in-person or virtually, through video or by phone.
With a blended approach of counseling techniques and evidence-based talk therapy, our behavioral health counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues. Vaccines
Vaccines are an essential part of public health. You have convenient access to many vaccines at Pipe Trades Family Health & Wellness Center. Your provider can determine which ones are right for you and your family based on your age, conditions, travel habits and other factors. You can then easily schedule an appointment or simply walk in to get your vaccines.
We offer a variety of routine vaccines to protect your health and community, including Influenza, Hepatitis A, Hepatitis B, Human Papilloma Virus (HPV), MMR, Pneumococcal, Tdap/Td, Varicella and Zoster.

Care Consult - Coming Soon
If you receive an abnormal test result, are told by a provider in your community that you need major surgery or experience some other serious health issue, you may benefit from a second opinion. Your providers at Pipe Trades Family Health & Wellness Center can offer that through Premise Health’s partnership with Mayo Clinic. They’re able to virtually consult with specialists at Mayo to review your medical record and come up with a recommended treatment plan for your specific health concern. Your provider will then share the outcome of that conversation with you to help you navigate your care plan and any next steps.
Care Navigation - Coming soon
Pipe Trades Family Health & Wellness Center offers many services to cover your health needs. However, there may be times you need specialty care outside the center. Finding that care on your own can sometimes be confusing. That’s why we provide care navigation to support you in finding the affordable, high-quality care you need.
If your provider gives you a specialist referral, your care navigator will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your specialist visit to help with any ongoing care needs.
