You don’t have to coordinate your own care. With virtual primary care through Premise Health, you have access to a dedicated provider by phone or video. It’s just like seeing a primary care provider (PCP) in person, but without the waiting room, commute or disruption to your day.

Skip the various appointments at different offices and trying to communicate with multiple providers. Instead, designate one of our virtual primary care providers as your PCP. They’ll become your trusted health ally and help you better prioritize your health.

How virtual primary care is different

If you’ve used traditional telemedicine, you know what it’s like to repeat your health information at the beginning of each visit. It’s usually the provider’s first time meeting you, so they don’t have a thorough understanding of your medical history or your wellness journey.

When you use virtual primary care through Premise Health, your dedicated provider develops a trusting relationship with you. We not only treat the symptoms that caused you to request a visit, but also create a personalized care plan specific to you, so we can best monitor your health on a consistent basis.

Traci-lyn Eisenberg, DO, FACOFP
Medical Director, Digital Wellness Center

Dr. Traci-lyn Eisenberg is a proud board-certified osteopathic family physician and a fellow of the American College of Osteopathic Family Physicians (ACOFP). She is currently the medical director of the Digital Wellness Center. Originally from New Jersey, she attended medical school at Nova Southeastern University College of Osteopathic Medicine and finished her residency at Broward Health Medical Center. Dr. Eisenberg is passionate about family medicine and osteopathic medicine, and is the Immediate Past President of the Florida Society of the American College of Osteopathic Family Physicians and a governor for the ACOFP. She takes a special interest in serving the needs of the LGBTQI+ community, and is comfortable with PrEP and HIV medicine, but is well versed in comprehensive primary care. 

In her spare time, Dr. Eisenberg enjoys running and recently completed her fourth marathon. She loves traveling, and is always planning her next great adventure.

Heidi Morris, DO, MHA, FAAFP
Physician, Virtual Health

Dr. Heidi Morris has 30 years of clinical experience as a Family Physician. Dr. Morris is board-certified in family medicine. As a graduate of Ohio University College of Osteopathic Medicine, Dr. Morris served as an Assistant Dean for OUCOM. She has completed a Master of Health Administration and two fellowships including one in health policy, which has pushed her to embrace what she feels is the direction of medicine today – virtual health. 

Dr. Morris has two daughters in college, two dogs, two collard doves and two grand dogs that she is devoted to and loves very much. She enjoys cooking and baking with her daughters at home. She also considers shopping an athletic sport!

Maria Pielaet, MD
Physician, Virtual Health

Dr. Pielaet graduated from UCLA with a Bachelor of Artis in Interpersonal Communications. She received her Doctorate of Medicine from UCLA in 1986, and completed a residency specializing in Family Medicine in 1989. She had a private family practice in South Lake Tahoe for 18 years, working in a full spectrum of family medicine, including Obstetrics. Dr. Pielaet started with Premise Health in 2008, where she served as an onsite physician and medical director.

Dr. Pielaet has an integrative, holistic and functional philosophy about the practice of medicine that includes nutrition, fitness and healthy supplements as part of a complete approach to prevention and treatment. 

Dr. Pielaet enjoys biking, hiking, fishing, cycling, and anything Disney. She also enjoys oil painting, loves to play guitar and sing, and is thrilled for every minute she gets to spend with her two grown sons.

Sharone Jensen, MD, MA
Physician, Virtual Health

Dr. Sharone Jensen is a board-certified Emergency Medicine Physician, providing compassionate care to her patients for over 20 years. She received her Doctor of Medicine at the University of Vermont College of Medicine and completed her residency in Emergency Medicine at Robert Wood Johnson University Hospital Systems campus in Camden, New Jersey. Dr. Jensen’s comprehensive background also includes a bachelor’s degree in Nutrition from the University of Minnesota and master’s degree in Physiology awarded by Columbia University. Prior to becoming a physician, Dr. Jensen served the community as a dietitian, cardiac rehabilitation specialist and fitness instructor. Drawing upon her multi-faceted education and training to provide excellence in patient care, Dr. Jensen has extensive experience in the disciplines of Emergency Medicine, Urgent Care, Oncology Urgent Care, Primary Care/ Preventive Medicine and Pain Management.

In her free time, she enjoys long walks with her German Shepherd mix Gracie, as well as following various collegiate athletic activities, especially football and hockey. Dr. Jensen also enjoys spending time with friends and family and cooking for all!

Perry Funk, DO, AAHIVS
Physician, Virtual Health

Perry Funk is a board-certified osteopathic family medicine physician. Born and raised in northeast Ohio, he graduated from Ohio State University before obtaining his medical degree from Ohio University College of Osteopathic Medicine. He has practiced in Ohio for most of his career, including eight years at the Cleveland Clinic, before joining Premise Health in 2020.

Dr. Funk enjoys taking care of all age groups, with a focus on working together with patients to maintain a healthy life. He is married and loves spending time with his 3 dogs, bicycling, traveling, cooking, and exploring new restaurants.

Asha Robinson-Parks, MD
Physician, Virtual Health

Dr. Asha Robinson-Parks is a seasoned, board-certified Family Medicine Physician with a depth of experience treating newborns through geriatric patients. Upon completion of her residency at Georgetown University, she served as the Medical Director and Clinical Instructor in underserved communities in Washington, D.C. and Maryland. Providing timely and appropriate access to telemedicine has been a priority throughout her career. As a detail-oriented physician, she strives to discover the subtleties of member needs and to empower members to take active roles in pursuing their unique wellness identities. She values collaboration and a shared commitment.

Luis A. Mojicar, MD

Luis A. Mojicar, MD is a board-certified Family Physician with a fellowship training in Primary Care Sports Medicine. Dr. Mojicar has 24+ years of experience in medical practice. He has held roles to include Associate Professor, Medical Director, Staff Physician and Telehealth Physician. Dr. Mojicar has been with Premise Health (or a predecessor company) since 1980.

Neha B. Shah, MD
Physician, Virtual Health

Dr. Neha B. Shah is a board-certified Family Medicine Physician. She received her medical degree from the University of Alabama School of Medicine in 2008 and completed her residency in Denver, Colorado in 2011. She has practiced both hospital and outpatient medicine with an interest in health policy and access to care and finds telemedicine the best way to meet patients where they are and minimize barriers to healthcare. Dr. Shah is passionate about evidence-based medicine and thoroughly enjoys creating meaningful provider-member relationships that she believes are the key to effective and quality medical care. Further, she strives to engage patients in their own preventive care and wellness and prioritizes the person not the disease. 

When not working, she enjoys spending time with her family, traveling, and Alabama football!

Ahmad Saab, MD, MPH, FAAFP
Physician, Virtual Health

Dr. Ahmad Saab is a board-certified Family Physician. He completed his Family Medicine Residency through State University Medical Center at Stony Brook University in New York. He has a master’s degree in Public Health. Dr. Saab has 40 years of national and international experience as a Family Physician, Telemedicine Physician, Telehealth Physician and Clinical Informatics Consultant. His experience also includes acting Director of Clinical Informatics and previous Chairman of Family Medicine. Your wellbeing is his number one priority.

Timothy O’Donnell, MD, MPH, DIP-ABLM
Physician, Virtual Health

Dr. Timothy O’Donnell is a board-certified Family Medicine physician and leader of the Lifestyle Medicine specialty in Indiana. He founded the first private Indiana medical practice focused on Lifestyle Medicine which aims to improve health through lifestyle changes based on evidence. He is currently a faculty member for the IU School of Medicine and teaches Wellness, MindBody Medicine, Lifestyle Medicine, and clinical skills to 1st and 2nd year medical students. He has broad experience in hospital based primary care, corporate medicine, and as a small private practice founder and physician. He has a past career as a Music Technologist and Professor at Indiana University in Bloomington after receiving a Masters of Fine Arts in Music from University of California at Irvine. Dr. O’Donnell enjoys backpacking, arts, cooking, researching, and teaching about healthy behaviors and spending time with family.

Ed Schwartz, MD
Physician, Virtual Health

Dr. Schwartz completed his undergraduate training at the University of Cincinnati in 1987 with a BS in Biology.  He then worked in the pharmaceutical and medical equipment arena for 3 years before starting medical school at Wright State University School of Medicine where he graduated in 1995.  He then moved to Phoenix Arizona to complete his residency in Family Medicine at Phoenix Baptist Family Medicine Program.  Since that time, he has remained a board-certified Family Medicine Physician.  His career has involved working both in direct delivery of both acute and primary care working in both Emergency Medicine, Urgent Care, and Family Practice.  He has been fortunate to hold leadership positions within a large medical group in Arizona, working for health plans and in onsite since 2007.  Dr Schwartz currently is National Medical Director of Clinical Affairs for Premise Health along with working overnights in Virtual Primary Care.

Anthony Dallas, MD, MRO, FAAFP
Physician, Virtual Health

Dr. Anthony Dallas is a board-certified family physician who has been practicing family medicine for over 40 years. He graduated from medical school at the University of Kentucky and completed his residency in Family Practice in 1979. Dr. Dallas previously served as the Chief Medical Officer with Care Here before the company merged with Premise Health, where he became Vice President of Medical Operations. In addition to his work with Virtual Primary Care, he is the Medical Review Officer for Premise Health. Dr. Dallas also sits on the board for the Nashville North Network and Synergy IPA, as well as the co-chairman of the Care Allied Accountable Organization Board of Directors.

Virtual primary care is a digital healthcare service through Premise Health. It offers management of your ongoing care needs as well as 24/7/365, on-demand access to care with a virtual primary care provider (PCP) from anywhere within the United States. You can even designate a PCP if you prefer to see the same provider on an ongoing basis. Yes. If you would like to see a provider now, start by logging in to your My Premise Health account through the My Premise Health app or at mypremisehealth.com. On your dashboard, select “Get Care Now.” From there, simply follow the prompts to see the next available provider as soon as possible.

Yes. One reason your employer partnered with Premise Health is to provide you with confidential care. We will never provide any of your healthcare information to your employer. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records.

Virtual primary care was designed to treat your preventive, short-term and long-term health needs. If you are experiencing a medical emergency, please dial 911 to get the medical attention you need.

Virtual primary care visits are just like in-office visits. You will discuss your medical needs with your provider, and they will diagnose and prescribe medications when medically appropriate. Virtual primary care providers do not prescribe controlled substances, non-therapeutic drugs or other drugs that can be harmful due to their potential for abuse.
