Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

The wellness center is staffed, managed and operated by Premise Health. Your medical staff includes a combination of a nurse practioner, condition management registered nurse/wellness coach, physical therapist, and medical assistant. Each staff member has been carefully screened and is qualified to deliver exceptional care. The wellness center is not staffed or managed by Fiserv or their associates in any way.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your health by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see your test results and more.

Eligibility and cost

The wellness center is available to any active Fiserv associate scheduled to work 30 or more hours per week- both Fiserv insured or those who are covered by a Fiserv medical plan. Spouses should call the wellness center at least 24 hours in advance of their appointment.

Full-time associates (30+ hours/week) enrolled in Fiserv or another group medical plan as well as Spouses and dependents 18 years and older enrolled in Fiserv’s insurance plan are eligible to use Fiserv Wellness Center – Alpharetta and Wellness Coaching. 

Full-time associates and dependents ages 2 and older are eligible to use Premise Health Wellness Center – Plano.

Preventive and wellness visits are 100% covered: annual physical, condition management, skin screenings, well woman exam, vaccines, biometric screenings, allergy injections, wellness coaching.


Sick/acute visits are a $40 flat fee  (applied toward your insurance deductible): colds, nausea/GI issues, back/ musculoskeletal pain, headaches, etc.


Physical therapy is a $45 flat fee (applied toward your insurance deductible)


Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP.


Once you create a My Premise Health account, you can easily schedule a visit through the My Premise Health app or online at Simply log in, select “Schedule an Appointment” and choose your appointment type.

You can also call your wellness center or visit during regular center hours to make an appointment.

Virtual visits

To meet virtually with one of the providers at your wellness center, you must first set up an account on the My Premise Health app or online at if you haven’t already. Then, log in and select “Schedule an appointment.” Choose your appointment type that says “Video” or “Phone” next to it. You will then follow the prompts to complete scheduling.

A virtual visit it is just like in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need.


Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.


Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

The Wellness Center provides annual physicals, sick visits, chronic condition management, biometric
screenings, lab draws, allergy and flu shots, immunizations, urgent care, women’s health, physical
therapy, wellness coaching and condition management.

Lifestyle medicine is an evidence-based approach to preventing, treating and even reversing preventable chronic conditions by replacing unhealthy behaviors with positive ones – such as eating healthfully, being physically active, managing stress, getting outdoors, maintaining healthy relationships, avoiding risky substances, and getting adequate sleep. Premise providers are Lifestyle Medicine certified practitioners.

Physical therapy is care delivered by a licensed physical therapist aimed at easing pain and helping members function to the best of their ability. Physical therapists help members improve their quality of life through prescribed exercise, hands-on care and patient education.


Treatment plans address mobility, reducing or managing pain, restoring function and preventing disability. We also offer virtual ergonomic assessments and musculoskeletal (MSK) exams – a comprehensive analysis of your body that will provide you with a set of scores showing any risk factors for injury to your neck, back, arms and legs.

Wellness coaching is a unique behavior change process designed to help members achieve lasting
lifestyle change in the areas of weight management, nutrition, exercise, sleep, smoking cessation,
work/life balance and life satisfaction. Our Wellness Coaches are trained specialists who can help
members move toward better health at a comfortable pace.
