General

Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

The wellness center is staffed, managed and operated by Premise Health. Your medical staff includes a combination of a nurse practioner, condition management registered nurse/wellness coach, physical therapist, and medical assistant. Each staff member has been carefully screened and is qualified to deliver exceptional care. The wellness center is not staffed or managed by Fiserv or their associates in any way.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your health by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see your test results and more.

Eligibility and cost

  • Fiserv Wellness Center – Alpharetta: Yes, if you are a full-time employee working 30 or more hours per week and enrolled in a group medical plan you can use the onsite wellness center located in Alpharetta.  This site is also available to dependents age 18 and up who are enrolled in a Fiserv medical plan.
  • Premise Wellness Center – Plano: Yes, if you are a full-time employee working 30 or more hours per week and enrolled in a group medical plan you can use the nearsite wellness center located in Plano, TX.  This site is also available to dependents age 2+ who are enrolled in a Fiserv medical plan.
  • Virtual Wellness Center: No, you must be enrolled in a Fiserv U.S. Medical plan to access the Virtual Wellness Center.  This program is also available to dependents age 2+ who are enrolled in a Fiserv medical plan.
  • Nationwide Wellness Coaching: Yes, nationwide wellness coaching is available to all U.S. associates and their medically enrolled dependents age 18 and up. 

Fiserv Wellness Center – Alpharetta
Full-time associates who work 30 or more hours per week who are enrolled in a Fiserv medical plan or other group medical plan can use the onsite wellness center. This site is also available to dependents age 18 and up who are enrolled in a Fiserv medical plan.

Premise Health Wellness Center – Plano
Full-time associates who work 30 or more hours per week who are enrolled in a Fiserv medical plan or other group medical plan can use the nearsite wellness center. This site is also available to dependents age 2+ who are enrolled in a Fiserv medical plan.

Virtual Wellness Center

Fiserv associates enrolled in a U.S. Medical plan and their dependents age 2+ who are enrolled in a Fiserv medical plan can use the Virtual Wellness Center.

Wellness Coaching
Full-time associates who work 30 or more hours per week who are enrolled in a Fiserv medical plan or other group medical plan. Wellness coaching is also available to dependents 18 years and older of eligible employees if the dependent is enrolled in a Fiserv medical plan.

  • Preventive and wellness visits are 100% covered:
  • Sick/acute visits are a $40* flat fee: colds, nausea/GI issues, joint & muscle pain, headaches, rashes, etc.
  • Physical therapy (Alpharetta location only) is a $45* flat fee

(*applied to your medical plan deductible if you are enrolled in a Fiserv United Healthcare medical plan)

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Yes, you can request that your PCP share your records with your wellness center by completing your provider’s record sharing process. You can also transfer your records and designate a provider at your wellness center as your new PCP.

Scheduling

  • Fiserv Wellness Center – Alpharetta:  Once you create a My Premise Health account, you can easily schedule a visit through the My Premise Health app or online at mypremisehealth.com. Simply log in, select “Schedule an Appointment” and choose your appointment type. You can also call your wellness center or visit during regular center hours to make an appointment at (678) 375-7700.  While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time so you don’t have to wait in the wellness center. Same-day appointments are typically available.
  • Premise Health Wellness Center – Plano:  Once you create a My Premise Health account, you can easily schedule a visit through the My Premise Health app or online at mypremisehealth.com. Simply log in, select “Schedule an Appointment” and choose your appointment type. You can also call your wellness center or visit during regular center hours to make an appointment at (469) 545-1993.  While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time so you don’t have to wait in the wellness center. Same-day appointments are typically available.
  • Virtual Wellness Center:  Once you create a My Premise Health account, you can easily schedule a visit through the My Premise Health app or online at mypremisehealth.com. Simply log in, select “Get Care Now.” You can also call the Virtual Wellness Center to make an appointment at (888) 535-4979
  • Nationwide Wellness Coaching:  Contact our wellness coach at (678) 375-7700 to schedule your first appointment. Thereafter you can use the My Premise Health account to schedule appointments online.

Services

Physical therapy is care delivered by a licensed physical therapist aimed at easing pain and helping members function to the best of their ability. Physical therapists help members improve their quality of life through prescribed exercise, hands-on care and patient education.

Treatment plans address mobility, reducing or managing pain, restoring function and preventing disability. We also offer virtual ergonomic assessments and musculoskeletal (MSK) exams – a comprehensive analysis of your body that will provide you with a set of scores showing any risk factors for injury to your neck, back, arms and legs.

Wellness coaching is a unique behavior change process designed to help members achieve lasting
lifestyle change in the areas of weight management, nutrition, exercise, sleep, smoking cessation,
work/life balance and life satisfaction. Our Wellness Coaches are trained specialists who can help
members move toward better health at a comfortable pace.
