We build a trusting relationship with you, leading to better, quality care. With same-day or next-day appointments available, in-person or virtually, you can get the care you need when you need it. Provider Visits $40 Flat Fee*

This includes primary care visits for services such as:

  • Cold, flu or allergy symptoms
  • Nausea, vomiting or diarrhea
  • Muscle or joint pain
  • Rashes, insect bites and stings
  • Headache, earache, etc.
  • Testing: flu, strep, Covid-19
  • Urinalysis and urine pregnancy

Physical Therapy

$45 flat fee*

  • Physical therapy visits $45 flat fee
  • Common conditions that benefit include:
    • Shoulder, neck and back pain
    • Muscle/joint pain or impairment
    • Sprains and strains
    • Pre-and post-surgical conditions
    • Headaches (tension-related)
    • Carpal tunnel and sciatica
  • Free musculoskeletal wellness visit
  • Free virtual ergonomic assessments available nationwide

*Applied to your medical plan deductible if you are enrolled in a Fiserv UnitedHealthcare medical plan.

The care team you know and trust, available virtually

You can get the same quality care you receive at your wellness center, virtually. This convenient option gives you the flexibility to schedule a virtual appointment with your trusted provider during your center’s regular business hours, at the same cost as an in-person visit. You can talk to your provider by video or phone on the My Premise Health app or at Get the care you need at home, in the office or on the go.

A few reasons to book a virtual visit for your primary care needs*:

  • Follow-up appointment or to review test results
  • Cough, fever or sore throat
  • Nausea, vomiting or diarrhea
  • Cold, flu, COVID-19 or allergy symptoms
  • Stomachache, headache or earache
  • COVID-19 at-home testing

In addition to primary care, you can also be seen virtually for:

  • Virtual ergonomic assessment
  • Wellness coaching

*To conduct a local virtual visit, you must be in the same state in which your provider is licensed.

Donna McCloud-Davis FNP-BC/HC, Health Center Manager

Donna has been a Family Nurse Practitioner eight years. Prior to that she worked as an RN for 30+ years.  Her background is in emergency medicine, primary & urgent care and administration. Passion for Wellness & preventive medicine and is a lifelong learner.


Education: Donna obtained her BSN from Northeastern University, MSN from Emory University and Post-Master’s Certificate from Georgia Regent’s University.


Fun fact: Born and raised in Boston, MA. Donna has been married to Ron Davis for 5 years and enjoys her blended family of 3 sons (24, 31, 35), 2 daughters (20, 35) and 4 grandchildren (3, 6, 6,7).  She spends most of her spare time traveling.

Emily Tretinik, DPT Physical Therapist

Emily was called to physical therapy in the second grade at an elementary career day event. She followed her career goals and continues to find joy in helping others. She has a background in brain injury rehabilitation from Prisma Health as well as orthopedic rehabilitation from Emory Healthcare.

Education: Emily graduated cum laude from The University of Georgia and completed her post-graduate studies at Armstrong Atlantic University in consortium with the Medical College of Georgia. She obtained her Doctorate of Physical Therapy degree in 2010.

Emily finds value in listening, learning, and working together to accomplish goals and maximize outcomes.

Mary Thompson CCMA, CPT Medical Assistant

Mary is a certified clinical medical assistant and experienced phlebotomist. She has over 10 years of experience working in primary care, pediatrics, and neurology.

As a Fiserv associate, you have access to a comprehensive range of healthcare services, including primary care, condition management, lab services, wellness coaching and women’s health.
