Premise Health is the organization that operates and staffs the health centers. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

You will continue to receive low cost, high quality healthcare at the Shaw Family Health Center. Qualified preventive services, like annual wellness exams, are offered at no cost and covered 100%. Primary Care and Physical Therapy are $30. Wellness coaching, chronic disease management and nutrition appointments are free of charge to eligible associates and their covered dependents.

All associates and immediate family members in the same household. You do NOT have to be enrolled in the Shaw Medical Plan. Shaw associates and retirees, enrolled in Medicare beginning 1/1/25, are eligible for most services. We cannot provide services to anyone on Medicaid or family members with Medicare.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Adolescent children aged 12 through 17 require permission from a parent/guardian to activate their own My Premise Health account. You can call or visit your health center to give permission and obtain an activation link for an adolescent child.
Children under 12 are not eligible for My Premise Health accounts. However, parents/guardians can access their child’s medical record via My Premise Health. First, they must establish their own account and then select the “Request Minor Chart Access” form from the menu.

No. Due to the sensitive nature of medical information, each adult must have their own My Premise Health account. Once established, you may grant proxy access to each other using the “Grant Adult Chart Access” form within My Premise Health.

Shaw employees and dependents 18+ can sign up for a My Premise Health account at mypremisehealth.com or contact their Shaw Family Health Center to receive an activation code. Depending on the age of your dependent under the age of 18, their ability to create their own account and your access to their medical record varies as follows:
  • Dependents under 12 cannot have their own account. All medical records and appointments are managed by their parent/legal guardian.
  • Dependents aged 12–17 are considered adolescents and can have their own account, but they require parental/legal guardian permission aged 12 through 17 require parental/guardian permission to activate their own My Premise Health (MPH) account. Parents/guardians can obtain an activation link by calling their health center or by going to “Contact Support” on their MPH portal. This requirement is to comply with state and federal privacy laws. Yes, with the appropriate permissions. This is called proxy access and allows a parent/guardian (for minors) or designee (for adults) to log in to their personal My Premise Health account and then connect to information regarding their family member. You may request or grant proxy access by logging into your My Premise Health Account and selecting the desired action from the menu. You may also complete a paper consent at your health and wellness center.

There are two methods that can be followed to allow the parent/legal guardian full access to their adolescent’s account (ages 12 – 17):

The parent/legal guardian may contact their health and wellness center to authorize their teen to create their own My Premise Health account. The teen will receive a portal activation link via text or email. Once the account is activated, they may grant full access to their parent/legal guardian by selecting the “Grant Adult Chart Access” form from the menu.

The adolescent may sign an authorization for their parent/legal guardian to have full access to their My Premise Health account at their health and wellness center. This form must be scanned into the chart and then parent access will change from “Limited” to “Full.”

If you have not done so already, first set up your account on the My Premise Health mobile app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will put you in touch with your provider to begin your visit.

If you have proxy access to your family record, you can ask questions on their behalf from your access to their record. If you do not have proxy access, you should request access before communicating about them through My Premise Health. Any question you ask is added to your own personal health record. Consequently, communicating about another individual’s information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care for you or your family member.

To meet virtually with one of the providers at your health center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.
