Primary Care

Available at all locations.

We offer primary care services that include sick and preventive care, as well as chronic condition management. Our lifestyle approach addresses factors like sleep, nutrition, stress, and movement to promote long-term wellness.

Be Well Condition Management

Available at all locations.

Our Be Well Program provides condition management or supplemental care for high blood pressure, diabetes, asthma, COPD, and high lipid levels. As a participant, you will meet regularly with a registered wellness coach or care manager to personalize your treatment plan. For those enrolled in the Shaw Medical Plan: you pay $0 on approved prescription drugs (regardless deductible).

Behavioral health

Available at all locations.

Your behavioral health refers to habits and actions that affect your overall well-being, like eating, sleeping, or managing stress. It is closely linked to mental health because feeling anxious, stressed, or depressed can make it harder to maintain healthy behaviors.

Shaw believes your mental and emotional wellbeing are just as important as your physical health. Regardless of enrollment in the Shaw Medical Plan, all associates and household members can receive 5 FREE visits per life event or season*. A licensed clinical social worker can help you address a range of issues, including anxiety, depression, stress, PTSD, ADHD, grief, and more. If in-person visits isn’t for you, take advantage of 24/7 access to virtual visits.

*Visits are only $30 per session moving forward.

Provider-dispensed medications

Available at Andalusia location.

Skip the extra trip to the pharmacy. You can get your medications directly from your provider during a visit for as little as $7 per 30-day supply. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.

We carry close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.

Physical therapy

Available at the Dalton location; Receive 8 visits without a referral.

Our physical therapists work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.

We offer in-person and virtual services, including:

Therapeutic treatment for sports-related injuries
Neck or back pain
Arthritic diseases and conditions
Hip, knee, shoulder, foot and ankle rehabilitation
Postural or ergonomic retraining

Lab services

Available at all locations.

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Vaccines

Available at all locations.

Vaccines are crucial for public health. At Shaw Family Health Center, you can easily get various vaccines at low or no cost. Your provider will recommend the right ones for you based on several factors.

Human Papilloma Virus (HPV)
COVID-19
Measles, Mumps and Rubella (MMR)
Pneumococcal (Pneumonia, Meningitis, etc.)

Wellness coaching

Available at all locations.

Wellness coaching can help you achieve lasting lifestyle change in the areas of weight management, nutrition, physical activity, sleep, stress, smoking cessation, work/life balance and life satisfaction. Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and can take place virtually or in person.

Nutrition

Available in-person in Dalton and virtually at all locations.

Eating well is crucial to maintaining your overall health. Our care team uses medical nutrition therapy to help treat or prevent a range of health conditions such as diabetes, high blood pressure and heart disease, as well as address many digestive disorders and weight-related issues. We will tailor your care to your individual needs and come up with personalized wellness plans that factor in your lifestyle, risk factors and family history.

We can provide advice and ongoing education through one-on-one sessions, individual nutrition assessments, group nutrition classes, lunch and learns, seminars, self-guided nutrition tools and virtual appointments. Our goal is to give you a new appreciation for nutritious food, and in turn, improve your overall health.

Care Consult

Available at all locations.

If you receive an abnormal test result, are told by a provider in your community that you need major surgery or experience some other serious health issue, you may benefit from a second opinion. Your providers at Shaw Family Health Center can offer that through Premise Health’s partnership with Mayo Clinic. They’re able to virtually consult with specialists at Mayo to review your medical record and come up with a recommended treatment plan for your specific health concern. Your provider will then share the outcome of that conversation with you to help you navigate your care plan and any next steps.

Care Excellence

Available at all locations.

We make sure you’re never alone in making complicated decisions about your health. If you’re struggling to get a diagnosis or battling a rare, complex condition, we can help. Our partnership with Mayo Clinic offers you access to highly specialized care.

If medically appropriate, your provider will refer you to the Mayo Clinic for evaluation and/or specialized treatment. A Mayo specialist and your provider will work closely to coordinate your ongoing care.

Should you need to go to the Mayo Clinic, all travel and lodging are conveniently arranged for you and one caregiver. All costs associated with the Mayo Clinic Care Excellence program, including services, travel and/or lodging are covered for those enrolled in the Shaw Medical Plan. For those not covered by the Shaw Medical Plan treatment and/or travel expenses will be subject to the insurance carrier’s guidelines.

Care Excellence

Premise Health’s Care Excellence program provides members access to the Mayo Clinic Complex Care Program, which offers enhanced benefits to eligible members facing complex, rare or undiagnosed health conditions. These individuals may be eligible to be referred to Mayo Clinic for evaluation and/or treatment. If you have a complex or rare condition, or you have been unable to find answers from other medical providers, the Mayo Clinic Complex Care Program may be able to help you get the care you need.

Occupational health

Available at all locations.

Keeping you safe at work is a top priority. Your providers at Shaw Family Health Center are here to treat work-related injuries and illnesses to help you get back to feeling well.

Our occupational health services are available for all Shaw associates and include injury diagnosis and treatment and musculoskeletal evaluations.

Care Navigation

Available at all locations.

There may be times you need specialty care outside of our center. Finding that care on your own can sometimes be confusing. That’s why we offer care navigation services to support you in finding the affordable, high-quality care you need.

If your provider makes a specialist referral, our care navigation team will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your visits to help with any ongoing care needs.
