Our Be Well Program provides condition management or supplemental care for high blood pressure, diabetes, asthma, COPD, and and dyslipidemia (high lipid levels). As a participant, you will meet regularly with a registered wellness coach or care manager to personalize your treatment plan. For those enrolled in the Shaw Medical Plan: you pay $0 on approved prescription drugs (regardless deductible).

Your emotional wellbeing refers to habits and actions that affect your overall wellbeing, like eating, sleeping or managing stress. It is closely linked to mental health because feeling anxious, stressed or depressed can make it harder to maintain healthy behaviors.

Shaw believes your mental and emotional wellbeing are just as important as your physical health. Regardless of enrollment in the Shaw Medical Plan, all associates and household members can receive 5 FREE visits per topic or diagnosis each year.* A licensed clinical counselor can help you address a range of struggles, including anxiety, depression, stress, PTSD, ADHD, grief and more. If in-person visits aren’t for you, take advantage of virtual visits from 7:30 a.m. to 7:00 p.m. CT.

*After the five free visits, each visit is $30 per session.

Virtual Mental Health
You can also connect to licensed counselors in all 50 states by video or phone for convenient, personalized mental health care through My Premise Health. They’ll also work closely with the rest of your care team when needed to support your overall wellbeing.

Skip the extra trip to the pharmacy. You can get your medications at little cost ($7 per 30-day supply) directly from your provider during a visit. Schedule an appointment, discuss your health needs, and leave with your medication in hand.

We carry close to 200 medications, including over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions. When you’re about to run out of your medication, schedule a follow-up visit with your provider.

Visits are $30 and you can receive 8 total visits without a referral.

Keeping you safe at work is a top priority. We offer a full range of occupational health services to treat work-related injuries and illnesses and get you back to feeling well.

We offer many lab services to support your needs. We can perform onsite testing or collect and send samples to an offsite lab. We can also process labs ordered by providers outside of the Shaw Family Health Center. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider. Labs are $3.

Vaccines are an essential part of public health. Your care team can determine which ones are right for you based on your age, health conditions, travel plans and other factors.

Nutrition is key to better health, and we’re here to make it work for your life. Using medical nutrition therapy, we can treat and prevent conditions like diabetes, high blood pressure, heart disease, digestive issues and weight concerns. Your care plan will be personalized to fit your lifestyle, taste preferences, goals and health history.

We can provide advice and ongoing education through one-on-one sessions, individual nutrition assessments, group nutrition classes, lunch-and-learns, health fairs, seminars, podcasts, self-guided nutrition tools, and virtual appointments.

We know how important your child’s health is, so we’ll make sure they get the care they need as they grow. Our providers will support you and your family ages 2+, with comprehensive care for their health and wellbeing.

Premise Health’s Care Excellence program provides members access to the Mayo Clinic Complex Care Program, which offers enhanced benefits to eligible members facing complex, rare or undiagnosed health conditions. These individuals may be eligible to be referred to Mayo Clinic for evaluation and/or treatment. If you have a complex or rare condition, or you have been unable to find answers from other medical providers, the Mayo Clinic Complex Care Program may be able to help you get the care you need.
