These health centers are another key benefit of the Welfare Fund, which continues to improve and provide complete healthcare benefits for active and retired members eligible under the Midwest Operating Engineers Welfare Fund.

Premise Health is the organization that operates and staffs the health centers. We are one of the nation’s leading providers of direct healthcare. We are here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

After you have been treated at our health center​s, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Our health center​s do not have a full‑service pharmacy; however, we do keep a variety of medications on hand for the treatment of illnesses such as sinus infections, strep throat and more. Limited starter generic medications are provided as part of your visit to the health centers in support of care management (for example, a cholesterol lowering medication for the diagnosis of high cholesterol). Our providers are able to prescribe medications and send the prescriptions to your regular pharmacy for pickup at your convenience.

Visits are free of charge for eligible members and their dependents.

No. To use the health centers you must be eligible for benefits under the MOE Health and Welfare Fund.

Yes, as long as you remain eligible for benefits under the MOE Retiree Welfare Plan.

No. If you are not eligible for benefits, you may not use the health centers.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you are unable to keep your scheduled appointment, please call to cancel and/or reschedule.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
