Our health centers offer a wide range of services to meet your unique needs. Your providers will give you the convenient, quality care you need, all in one place – in person or virtually. They are also experts in coordinating care, which means they can refer you to specialists when needed and make sure nothing is overlooked.

Primary care
We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your chronic conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up.
Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management
You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with personalized treatment plans to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, weight control, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.
Mental health
We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Your licensed counselor will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.
With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Provider-dispensed medications
Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
Our health centers carry close to 100 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.
Women's health
We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. Along with general primary care services, we offer the following specifically for women’s health:
- Breast cancer screenings

- Cervical cancer screenings (Pap smears)

Physical therapy
You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your wellbeing. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.
We offer a full range of in-person and virtual services, including therapeutic exercise and conditioning, pre- and post-operation rehabilitation, ergonomics evaluations, orthopedic rehabilitation, health management and awareness education.

Lab services
We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.
We can perform and process the following onsite:

Vaccines
Vaccines are an essential part of public health. You have convenient access to many vaccines at our health centers. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.
We offer a variety of routine vaccines to protect your health and community, including:
- Influenza (Flu)
- Hepatitis A
- Hepatitis B
- Human Papilloma Virus (HPV)
- Measles, Mumps and Rubella (MMR)
- Pneumococcal (Pneumonia, Meningitis, etc.)
