*This location offers primary care, lab services, physical therapy, chiropractic care, mental health, wellness coaching and care management.

Kate Hymes, FNP-C – Nurse Practitioner
Kate Hymes, FNP-C is a full-time family nurse practitioner at Premise Health Northbrook. She holds a national certification with the American Academy of Nurse Practitioners.
Patient Care Philosophy:
Her approach to care is patient-centered, encouraging patients to take an active role in their health care by empowering them to make evidence-based choices. Kate believes that the ability to make sustainable changes begins with education and options, tailored to individual patient healthcare needs.
