Johann Serag, MD – Medical Director

Dr. Johann Serag is our newest medical director at Premise Health Wellness Center Northbrook.  He brings along over a decade of experience in the practice of Family Medicine and has been board certified since 2012.

 

Patient Care Philosophy: 

He believes in the saying “Wherever the art of medicine is loved, there is also love for humanity.”  He looks forward to partnering with all members to manage their health wherever they are in their healthcare journey. 

 

How I stay well:

During his free time he does outdoor activities like hiking, biking, golf and most recently pickleball. 

Carla Johns, FNP – Nurse Practitioner

Carla Johns has been a nurse practitioner for the past 16 years including 11 years with Premise. Her clinical interests include helping patients meet their lifestyle goals and reducing medication burden.

Patient Care Philosophy:

She practices with shared decision-making in mind, valuing patient’s input in their care.


How I stay well:

When not at work, she is busy with her 3 children and enjoys attending CrossFit classes and travel with hopes to visit all seven continents.

Kate Kroschel, FNP-C – Nurse Practitioner

Kate Kroschel, FNP-C is a full-time family nurse practitioner at Premise Health Northbrook. She holds a national certification with the American Academy of Nurse Practitioners.

 

Patient Care Philosophy:  

Her approach to care is patient-centered, encouraging patients to take an active role in their health care by empowering them to make evidence-based choices. Kate believes that the ability to make sustainable changes begins with education and options, tailored to individual patient healthcare needs.

  

How I stay well:  

In her free time, Kate enjoys camping, jogging, and outside music events.

Camilla Philippe, LCSW – Licensed Clinical Social Worker

Camilla received her Bachelor’s in Social Work from Western Illinois University and completed her Master’s in Social Work at Aurora University. She has worked extensively in a multitude of settings including mental health facilities, nursing homes, and homeless shelters. Camilla has been in social work for about 10 years.   

Camilla has experience working with children, adolescents, adults, families, and couples. She specializes in anxiety, depression, stress management, relationship concerns, low self-esteem, and chronic mental illness. Camilla uses Solution Focused treatment, Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and Strengths based approaches as my treatment approaches.

 

Patient Care Philosophy:

She is compassionate, non-judgmental, and an empathic listener. She believes in personal growth and a collaborative therapeutic relationship to create treatment plans to assist and support her clients in achieving their goals.
