When are the UBS Employee Health Centers open?
The hours of operation for all locations are:
Monday – Friday, 8 a.m. – 4 p.m.
Closed 12:30 – 1 p.m. for lunch
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.
How much does a visit cost?
Visits will incur a charge of $55-$100 (depending on location) at the time of service until your deductible is met. Once the deductible is met, the plan covers any visits as an in-network service.
If I do not have insurance through UBS, can I still use the center?

Will my current medical provider or PCP have access to my health records if I receive care at the health center?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Who is eligible to use the UBS Employee Health Centers?
The UBS Employee Health Center is available to all benefits-eligible employees, regardless of their election in the UBS benefits program.
Is my health information secure? Does UBS have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
