Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pregnancy testing, pelvic exams
  • Contraception counseling
  • STD testing and counseling
  • Cervical cancer screenings (Pap smears)
  • LGBTQ+ care
  • Education for self-awareness

Mental health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Your licensed behavioral health providers will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone. 

With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues. 

You’re not alone. We’re here to help. Get the support you need at the UBS Employee Health Centers, offered currently at the Weehawken, New York (1285 Avenue of the Americas and Madison Avenue) and Nashville locations.

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following onsite:  

  • Glucose or lipid testing
  • hemoglobin A1C 
  • Rapid strep testing 
  • Flu testing 
  • Urine pregnancy testing 
  • Urine dip screenings 
  • Rapid mono testing
  • COVID-19 testing
  • Community provider ordered lab testing

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at the UBS Employee Health Centers. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community. Check your local health center to see what they offer.

Travel medicine

At the UBS Employee Health Centers, we offer travel medicine services to help you plan for your trip and to stay well while you’re away. Depending on your destination, getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment, or specialist referral.

Physical therapy

You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your wellbeing. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.

Our Madison Ave. location offers a full range of in-person and virtual services, including:

  • Neck related and tension headaches
  • Muscle aches and knots
  • Joint stiffness and mobility issues
  • Plantar fasciitis
  • Lower back pain
  • Neck pain
  • Rotator cuff injuries
  • Post surgical rehabilitation
  • Injury prevention

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

Get the same great care by phone or video.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Premise Health is the organization that operates and staffs your health centers. We are the nation’s leading provider of direct healthcare, partnering with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

The UBS Employee Health Centers are staffed, managed and operated by Premise Health. The medical staff includes physicians, nurse practitioners, physician assistants, licensed clinical social workers, physical therapists, and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by UBS or their employees in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Eligibility and cost

The UBS Employee Health Center is available to all benefit’s eligible employees, regardless of their election in the UBS benefits program.

Visits will incur a charge of $65-$100 (depending on location) at the time of service until your deductible is met. Once the deductible is met, the plan covers any visits as an in-network service.

While where you receive care is a personal choice, we hope you’ll consider trusting us with your overall wellbeing, much like UBS has trusted us as a partner in health. We’d love to see you soon!

The UBS Employee Health Centers are available to all benefits-eligible employees, regardless of their election in the UBS benefits program.

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

You can also call your wellness center or visit during regular center hours to make an appointment.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.
