At Premise Health, we believe in better care. We offer members like you a comprehensive range of services to cover your needs, including primary care, occupational health, vaccines and more. Our providers are committed to helping you get, stay and be well.

Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Occupational health

Keeping you safe at work is a top priority, which is why we offer a full suite of occupational health services. Your providers at Roche Wellness Center are here to treat work-related injuries and illnesses to help you get back to feeling well.

Our occupational health services are available for all Roche Employees and include injury/illness diagnosis and treatment, emergency care services, musculoskeletal evaluations, and for-cause drug and alcohol testing.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling
  • STD testing and counseling
  • LGBTQ+ care
  • Hormone therapy
  • Education for self-awareness

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at Roche Wellness Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors. 

We offer a variety of routine vaccines to protect your health and community, including:

  • Influenza (Flu)
  • COVID-19
  • Hepatitis B
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Zoster (Shingles)

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider. 

We can perform and process the following onsite:

  • COVID-19
  • Glucose or lipid testing 
  • Rapid strep testing 
  • Flu testing 
  • RSV testing
  • Pregnancy testing 
  • Urine screenings 
  • Rapid mono testing
  • Hemoccult

Lab tests that are sent to Labcorp for offsite testing include the Bio360 panel, complete metabolic panel, complete blood panel, thyroid-stimulating hormone, vitamin D, HbA1c, and lipids. We also offer lab draw services for outside orders from community providers.

Wellness

Our wellness programs promote nutrition, physical activity, stress management, healthy relationships and more. We take a comprehensive approach that focuses on your overall health and wellbeing. Our providers will personalize your wellness plan and help you make positive lifestyle changes that can lead to a more balanced and healthier you.

Your wellness services include biometric screenings, wellness education, condition management and monthly wellness offerings/events.

Travel medicine

At the Roche Wellness Center, we offer travel medicine services to help you plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment, or specialist referral.

Nutrition

Eating well can improve your overall wellbeing. It can also help treat existing conditions and decrease future risk. At the Roche Wellness Center, your registered dietitians use medical nutrition therapy to help treat a range of conditions, such as diabetes, hypertension, cardiovascular disease, as well as many digestive disorders and weight-related issues. They will tailor your nutrition counseling to meet your unique needs and lifestyle with personalized action plans, based on your risk factors and family history.

Our registered dietitians can provide advice and ongoing education, in-person or virtually through one-on-one tailored sessions, individual nutrition assessments and dietitian-guided appointments to give you a new appreciation for healthy food, and in turn, improve your overall health.

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

Get the same great care by phone or video.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Premise Health is the organization that operates and staffs your wellness center. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

The Roche Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, physician assistants, nurse practitioners, registered nurses, and Medical Assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness centers is not staffed or managed by Roche Diagnostics or their employees in any way.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Eligibility and cost

All Roche Employees and their covered dependents (ages six and older) enrolled in a Roche United Healthcare employer-sponsored plan are eligible for services based on plan benefits.

Services at the Roche Wellness Center are available at zero cost to Roche employees. Insurance is utilized when needed. 

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need. 

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

Meet your care team

Rhonda Paris, FNP
Health Center Manager

Rhonda has been the Roche Wellness Center manager for over 10 years and a Family Nurse Practitioner for 24 years. She completed her undergraduate program and graduate program at Ball State University. Rhonda has a passion for people and partnering with people to live a healthy lifestyle. She loves the Roche culture and the commitment to their employees through many holistic benefits. She is very proud of the Live Well Programs. She has 3 kids. Traveling, hiking, exercising, and spending time with her family and friends is what she enjoys doing in her free time.

Brooke Southerland, PA
Physician Assistant

Brooke Southerland is a certified physician assistant. She graduated from Le Moyne College with a masters degree in physician assistant studies in 2011. After working many years in the emergency department, she loves providing preventative care to help people feel their healthiest selves. In her free time, Brooke likes to run a community garden that grows food for neighbors in need and hang out with her family and chickens. 

 

Marleah Money, MA
Medical Assistant

Marleah has been a certified Medical Assistant for over 10 years and has been in the customer service industry for over 15 years. In her free time, she enjoys hikes and time with her family.  

Krista Armstrong, FNP
Family Nurse Practitioner

Krista has been in her current position at Roche for the past 13 years and in health care for a total of 21 years.  Before her role at Roche, she worked as a nurse in Labor and Delivery and PICU, then as a Nurse Practitioner in the emergency department. Her undergraduate degree is from Valparaiso University and her master’s degree is from IUPUI.  

What she likes most about the Roche Wellness Center is the opportunity to get to know Roche employees and take the time to understand their healthcare needs.  She also loves the emphasis that Roche puts on wellness and disease prevention. She is grateful for the opportunity to work alongside the care team at the Roche Wellness Center to help employees achieve their goals.  

In her spare time, you will likely find Krista in a fitness class, outside walking (listening to a good podcast or audiobook), or spending time with family and friends.

Jill Fitzpatrick, FNP
Family Nurse Practitioner

Jill obtained her Family Nurse Practitioner Certification in 2011 from IU after working many years as a nurse at IU Health Methodist Trauma Center. Prior to joining the Roche Wellness Center team in 2015, Jill worked as a Family Nurse Practitioner at IU Health West Emergency Department. 

Jill’s passion for preventive medicine led her to change career paths from emergency medicine to primary care. Jill enjoys working with Roche employees to improve their health and prevent chronic disease. 

When Jill is not in the office, she enjoys spending time with her husband and three beautiful daughters. She enjoys cheering her girls on during their activities, exercising, and traveling.

Valerie Turner
Phlebotomist

Valerie is a phlebotomist and has been a member of the care team at the Roche Wellness Center since 2010. She graduated from Martin University with a BA in Biology and earned an associate’s degree in medical assisting from Kaplan College. Valerie has over 25 years of experience in healthcare. Before joining the Wellness Center, she was a lab associate specializing in quality assurance and establishing quality control measures. 

She enjoys the work environment at Roche Wellness Center and spends her free time traveling, shopping and collecting antiques.

Lauren Carroll, RN
Registered Nurse

Lauren is a registered nurse with years of experience in an occupational health setting. She earned a Bachelor of Science in Psychology from St. Joseph’s College and a Bachelor of Science in Nursing from Purdue University. 

Before her role at the Wellness Center, Lauren worked as a nurse at a light manufacturing facility in Lafayette, IN. She loves the people at Roche, and in her spare time, Lauren enjoys reading, nature, and French pastries.

April Rhymaun
Registered Dietician

April has been supporting Roche employees since September 2017. She has worked in various roles as a Registered Dietitian/Nutritionist since earning her Master’s in Nutrition and Food Science from Colorado State University in 2010. Before joining the Roche Wellness Center team, April’s work experience includes nutrition consulting in private practice, serving as the Dietetic Program Director and lecturer at Metropolitan State University of Denver, and as a Worksite Wellness Coordinator at Colorado Department of Public Health. Prior to becoming a dietitian, April worked as a CPA and has a Bachelor’s in Accounting from the University of Iowa.

What she likes most about Roche is supporting employees using a personalized approach to best meet their needs and preferences.

Her hobbies include all things outdoorsy and spending time with her husband and daughter.
