Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Do the health centers have a pharmacy?
Our health centers don’t have a full‑service pharmacy; however, we keep a variety of medications on hand for the treatment of illnesses such as sinus infections, strep throat and more. Limited starter generic medications are provided as part of your visit to the health centers in support of care management (for example, a cholesterol lowering medication for the diagnosis of high cholesterol). Our providers can prescribe medications and send them to your regular pharmacy for pickup.
How much does a visit cost?
Visits are free of charge for eligible members and their dependents.
Can I use the health centers if I’m not eligible for MOE Health and Welfare Benefits?
No. To use the health centers, you must be eligible for benefits under the MOE Health and Welfare Fund.
Can I continue to use the health centers when I retire?
Yes, as long as you remain eligible for benefits under the MOE Retiree Welfare Plan.
Can I use the health centers if I’m not eligible for benefits but willing to pay for services?
No. If you’re not eligible for benefits, you may not use the health centers.
How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.
