We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you and your eligible dependents so they are better equipped to prevent, treat and manage any health issues that may come up.
Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.
Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
CTK Health and Wellness Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.

Our turnaround time for processing routine lab tests is typically 24 to 48 hours. Our Centralized Virtual Wellness Coaching can help you achieve lasting lifestyle change in the areas of weight and stress management, nutrition coaching, physical activity routines, tobacco cessation, work/life balance and life satisfaction.
Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and take place virtually.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.
A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.
CTK Health and Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, nurses, medical assistants, and virtual wellness coaches. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by Meyer Tool or their employees in any way.
My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Fees depend on your health plan selection with your employer. For members with the HDHP there will be a $20 fee due at time of appointment. The health centers accept credit cards only. No cash or checks accepted.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Under current Kentucky state regulations, Nurse Practitioners are not permitted to dispense medications directly from our onsite medication dispensary. However, Nurse Practitioners are fully authorized to provide high-quality care within our health centers. If medication is deemed necessary as part of your treatment, they can issue a prescription and send it to your preferred local pharmacy. Ask your provider about our Home Medication Delivery program for chronic condition medications.
Premise Health has partnered with The Little Clinic to provide care for sick visits after hours or if your health center is full. Simply call the patient support center (877) 685-5883 OR use the chat feature to utilize this service.
Here’s how to secure your visit with The Little Clinic through Premise Health:
1) Call our Patient Support Center at (877) 685-5883 or use the Premise Health Chat feature to attempt to schedule an appointment with your health center.
2) If your health center is closed or full for the day, ask the Patient Support Center Advocate to secure your visit with The Little Clinic location of your choice in your area.
3) Go to your chosen The Little Clinic location and advise the staff that Premise Health sent you and proceed with your visit! Your spot will be secured for the business day.
