Your primary care provider will get to know you and your eligible dependents, so they’re better equipped to prevent, treat and manage any health issues that may come up. The best part? Women's health

Women have unique healthcare needs. Our providers spend time getting to know you, or the women in your life, to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health.

Along with general primary care services, we offer the following specifically for women’s health: breast cancer screenings, pregnancy testing, pelvic exams, contraception counseling, STD testing and counseling, cervical cancer screenings (Pap smears), counseling and referrals for interpersonal and domestic violence, LGBTQ+ care, comprehensive family care, and education for self-awareness. Women’s health services are available for all employees and dependents Jackson Rancheria’s Medical Plan.

Condition management

You don’t have to navigate your health alone. With our condition management services, you and your eligible dependents can get the support you need to manage your conditions and take control of your health. Our providers will come up with personalized treatment strategies to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide disease specific education, condition management tactics, diet guidelines and physical activity goals, medication counseling and preventive and ongoing monitoring to manage your conditions and help you and your family stay well.


Vaccines are an essential part of public health. You have convenient access to many vaccines at the Jackson Rancheria Medical Center. Your provider can determine which ones are right for you and your eligible dependents based on your age, conditions, travel habits and other factors. You can then easily schedule an appointment or simply walk in to get your vaccines.

We offer a variety of routine vaccines to protect your health and our community, including Influenza, Hepatitis A, Hepatitis B, Tdap/Td, and Shingrix.

Lab services

To support you and your eligible dependent’s needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following lab tests onsite: glucose testing, rapid strep testing, influenza testing, urine pregnancy testing, urine dip screenings, and rapid mono testing. We also offer COVID-19 testing through your virtual care benefits. The kit is sent to your door through our lab services partner, LabCorp, and results come back within 72 hours.* Simply request a virtual visit to get started.

*Turnaround time for results is subject to national trends and case surges.

Occupational health

Keeping you safe at work is a top priority, which is why we offer a full suite of occupational health services. Your providers at the Jackson Rancheria Medical Center are here to treat work-related injuries and illnesses to help you get back to feeling well.

Our occupational health services are available for all Jackson Rancheria employees and include injury diagnosis and treatments, musculoskeletal evaluations, and return-to-work plans.

You can get the same great care you receive at your wellness center, virtually, either online or over the phone. This safe and convenient option gives you the flexibility to schedule a virtual appointment with your trusted provider during your center’s regular business hours, at the same cost as an in-person visit. Get the care you need at home, in the office or on the go.

Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

Jackson Rancheria Medical Center is staffed, managed and operated by Premise Health. Your medical staff includes a combination of a nurse practitioner and a medical assistant. Each staff member has been carefully screened and is qualified to deliver exceptional care. Jackson Rancheria Medical Center is not staffed or managed by Jackson Rancheria or their employees in any way.

Employees & dependents covered by Jackson Rancheria’s Medical Plan are eligible. Flu vaccines and at-home COVID-19 tests are available for all employees and their household members.

The cost of services, including labs and select dispensary medications, is covered by the Jackson Rancheria Casino Resort. You will incur a $50 charge, however, if you do not attend an appointment you scheduled without calling to cancel 24 hours in advance.

Employees & dependents 2 years of age and older covered on Jackson Rancheria’s Medical Plan are eligible. Flu vaccines and at-home COVID-19 tests are available for all employees and their household members.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. The easiest way to make an appointment is through your online member portal. Visit mypremisehealth.com to set up your account and then schedule appointments online or through your My Premise Health mobile app.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your health by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see your test results and more.

Yes, the Medical Center is always welcoming new members! You can be seen as a new patient for primary care or sick care needs, even if you have a primary care doctor in the community.

When you have your labs drawn, regardless of if they’re ordered by the Jackson Rancheria Medical Center provider or a provider in the community, you will not have to pay any out of pocket cost! Most labs can be drawn at the health center, and your orders will be reviewed by the medical assistant during scheduling. Then, simply bring the lab order from the community provider or specialist to your lab draw appointment!

Visits are available in 3 formats- in-person, via video, and telephone call. Video visits can be scheduled and accessed by going to mypremisehealth.com, or by downloading the My Premise Health app on your smart device. From there, you can login or register for a new account, schedule your visit, sign any required paperwork, and begin the visit!

Appointments are generally available on the same day or the next day!
