Premise Health is the organization that operates and staffs your health center. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
  • Preventive and wellness visits are 100% covered
  • Sick/acute visits are $40* (colds, nausea/GI issues, joint and muscle pain, headaches, rashes, etc.)
  • Physical therapy visits (Alpharetta location only) are $45*
  • Virtual visits are free (*Applied to your medical plan deductible if you are enrolled in a Fiserv United Healthcare medical plan)

Who is eligible to use the Wellness Center – Alpharetta, GA?
Fiserv associates who work 30 or more hours per week who are enrolled in a Fiserv medical plan or other group medical plan. Dependents of eligible associates who are age 18 and up who enrolled in a Fiserv medical plan.

Who is eligible to use Nationwide Wellness Coaching?
Fiserv associates who work 30 or more hours per week who are enrolled in a Fiserv medical plan or other group medical plan. Dependents age 18 years and older who also enrolled in a Fiserv medical plan.

Who is eligible to use Nutrition Services?
Fiserv associates who work 30 or more hours per week who are enrolled in a Fiserv medical plan or other group medical plan. Dependents age 18 years and older who also enrolled in a Fiserv medical plan. Nutrition services are available in the following states: AZ, CA, CO, FL, GA, MA, MI, NE, NJ, NC, OH, TX, UT, VA and WI.

Who is eligible to use the Virtual Wellness Center?
The Virtual Wellness Center is available to all associates enrolled in a Fiserv U.S. Medical Plan and their medically enrolled dependents ages 2 and up who are enrolled in a Fiserv medical plan.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

The Virtual Wellness Center is a digital healthcare service through Premise Health. It offers management of your ongoing care needs as well as 24/7/365, on-demand access to care with a virtual primary care provider (PCP) from anywhere within the United States. You can even designate a PCP if you prefer to see the same virtual provider on an ongoing basis. Virtual Wellness Center visits are available through the My Premise Health app, online at mypremisehealth.com or by phone at (888) 535-4979.
