Who is Premise Health?
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.
Where is the Eaton Health Center located?
Eaton Health Center:

The Health Center is located on the main floor on Wellness Blvd.
Eaton Fitness Center:
1000 Eaton Blvd.

Eaton Fitness Center is located past the Health Center at the end of Wellness Blvd.
When is the Eaton Health Center open?
Eaton Health Center
Monday – Thursday, 7:30 a.m. – 4:30 p.m.
Friday, 8 a.m. – 12 p.m.
Eaton Fitness Center
Monday – Thursday 6 a.m. – 7 p.m.
Friday 6 a.m. – 2 p.m.
How much does a visit cost?
- Costs for services at the Health Center vary depending on your medical plan, but visits are more cost-effective than services provided in the community
- Nutrition Services are free of charge
- Behavioral Health Services are free of charge
- Free Preventive Services: Annual Physical Exam, Travel Visits and Vaccinations
Who is eligible to use the Eaton Health Center?
Eaton Health Center:
All US Eaton employees and their dependents ages 2 and up are eligible to use the Eaton Health Center.
Eaton Fitness Center:
Employees at Eaton Center in Beachwood, Ohio or employees who are visiting Eaton Center in Beachwood, Ohio for business reasons are eligible to use the onsite fitness center.
How do I create an Eaton Fitness Center account?
To create an account, please visit https://eatonfitness.fitnessanalyst.com.

What services and programs are offered at the Eaton Fitness Center?
The Eaton Center Fitness Center offers free personal training, group exercise, and movement & performance assessments. Physical therapy and massage therapy are also available as fee for service offerings. You can also find full-service locker rooms complete with saunas and steam rooms.
How do I sign up for programs and services at the Eaton Fitness Center?
The best places to find Eaton Fitness Center programs and services is on our website https://eatonfitness.fitnessanalyst.com and in the Eaton Center Fitness Center Viva Engage community.
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
