Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management

You don’t have to navigate your health challenges alone. With our condition management services, you and your eligible dependents can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues. 

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling and preventive and ongoing monitoring to manage your symptoms and help you and your families stay well.

Provider-dispensed medications

Skip the extra trip to the pharmacy. You can get your medications at no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.

City of Sanford Employee Health Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support. 

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pregnancy testing
  • Pelvic exams
  • Contraception counseling
  • STD testing and counseling
  • Cervical cancer screenings (Pap smears)
  • Counseling for interpersonal and domestic violence
  • LGBTQ+ care
  • Comprehensive family care
  • Hormone therapy and education for self-awareness

Lab services

We offer many lab services to support you and your eligible dependent’s needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

  • Rapid strep testing
  • Influenza testing
  • Urine pregnancy testing
  • Urine dip screenings
  • Rapid mono testing
  • H-pylori testing

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at City of Sanford Employee Health Center. Your provider can determine which ones are right for you and your family based on your age, conditions, travel habits and other factors. 

We offer a variety of routine vaccines to protect your health and community, including:

  • Influenza
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • MMR
  • Pneumococcal
  • Tdap/Td
  • Varicella
  • Zoster

Occupational health

Keeping you safe at work is a top priority, which is why we offer a full suite of occupational health services. Your providers at City of Sanford Employee Health Center are here to treat work-related injuries and illnesses to help you get back to feeling well.

Our occupational health services are available for all City of Sanford employees and include injury diagnosis and treatment, emergency care services, musculoskeletal evaluations, ergonomic assessments, pre/post-employment exams, drug and alcohol testing, physical therapy and occupational therapy services and return-to-work plans.

Wellness coaching

City of Sanford Employee Health Center offers wellness coaching to help improve your overall wellbeing. Our certified wellness coaches will meet you where you are and help you make actionable goals to build new skills and make positive lifestyle changes. All coaching sessions are completely confidential and can take place virtually or in-person.

Our wellness coaches address a variety of lifestyle needs, including weight and stress management, nutrition coaching, physical activity routines, tobacco cessation, work/life balance and life satisfaction. They will help you reach your wellness goals through individual wellness coaching and goal setting, group education and lifestyle management programs, corporate challenges, and health and nutrition promotion through seminars, health fairs, meditations and podcasts. Wellness coaching is available virtually for all City of Sanford employees, spouses and dependents over the age of 8 that are on the City of Sanford’s insurance plan. 

My Premise Health

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Premise Health is the organization that operates and staffs your wellness centers. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

Sanford Employee Health Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurses, medical assistants, and wellness coaches. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by City of Sanford or their employees in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

You can also call (407) 513-4150 or visit during regular center hours to make an appointment.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.
