Who is Premise Health?
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Monday – Thursday: 7 a.m. – 5:30 p.m.
Friday: 8 a.m. – 5 p.m.
(Closed daily for lunch from 12 – 1 p.m.)
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How much does a visit cost?
Services are available at no cost.
Who is eligible to use the City of Ocala Employee Health Center?
Services are available to all employees, retirees, and dependents at least 2 years of age covered by the city’s insurance plan.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Is my health information secure? Does the City of Ocala have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. If I do not have insurance through the City of Ocala, can I still use the center?
No. To use the center you must be covered by the city’s insurance plan.
