Premise Health is the organization that operates and staffs the health centers. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Our parking lot is off of E 1st Ave, between the community garden and courthouse. We do share a building with Mesa Police Department.

Monday – Thursday, 7 a.m. – 6 p.m.
Friday, 8 a.m. – 4 p.m.

City of Mesa employees, their dependents, or retirees that are on the City of Mesa health plan (Cigna), are eligible to utilize the health center services.

Utilizing the City of Mesa Health and Wellness Center operated by Premise Health is cost effective. All services are at no charge. Lab, referral providers/services, and prescriptions are subject to insurance.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
