We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Calhoun County:
PPO Members: Nothing. All services and medications are at no cost to those eligible to use the Calhoun County Health & Wellness Center.
HDHP (HSA) Members: Preventive services and medications received from, or mailed through the health center, are at no cost to members enrolled in the HDHP. However, consistent with Federal rules and regulations for Health Savings Accounts (HSAs), there is a $25 Service Fee for Non-Preventive services. Service Fees are then applied to the member’s deductible.

Musashi: Nothing. All services and medications are at no cost to those eligible to use the Calhoun County Health & Wellness Center.

Systex: Nothing. All services and medications are at no cost to those eligible to use the Calhoun County Health & Wellness Center.

Calhoun County: All employees and dependents ages 2 years old and above who are enrolled in the County’s health plan are eligible to utilize the health center services.

Musashi: All employees and dependents ages 2 years old and above who are enrolled in the Employee Sponsored Health Plan are eligible to utilize the health center services.

Systex: All employees and dependents ages 2 years old and above who are enrolled in Systex’s health plans are eligible to utilize the health center services

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff.
