Weight is the result of many factors, from your lifestyle to your preferences to your medical needs. On top of that, if you have weight-related health conditions like diabetes, high blood pressure or high cholesterol, the journey to reaching your wellness goals can feel even more complex. Regardless of where you are on that journey or your health conditions, Beyond Weight is here to help.
Beyond Weight is a 12-month program that can help you reach your health and weight goals.
It looks past the scale and uses a personalized, comprehensive approach as well as education on nutrition, movement, weight loss medications and more. In addition to being personalized, Beyond Weight is also easy to access, as it’s offered both in person and virtually, depending on your wellness center.

How is Beyond Weight different?
This time, it's personal.
Weight loss is about so much more than counting points or cutting certain foods from your diet. Beyond Weight treats weight loss as a personal, comprehensive experience that includes:
- Lifestyle behavior change
- Meal planning
- Stress management
- Eating healthy on a budget
- Goal setting
- Stalls and plateaus with weight loss
- A care team to answer all of your health-related questions
This program uses your lifestyle, preferences and medical history to create a plan that will work for you in the long term. Rather than get black-and-white dieting advice, you’ll receive a personalized care plan that’s been created based on your unique health needs.
You’ll also get on-on-one support from a trusted provider every step of the way, so you’ll never feel like you’re in this alone.
What to expect
There's a lot to look forward to.
You’ll first visit with a Premise Health provider to ensure the program will be a good fit for you. If your provider believes Beyond Weight would help you reach your health and weight goals, they’ll have you start the program with a biometric screening. This will be a marker of where you are so you can accurately track your progress and celebrate your wins.
Before you’re enrolled, you’ll be asked to sign the Member Agreement and commit to regular weight reporting, quarterly visits with your provider and monthly visits with your nurse.
Your provider will develop a custom program based on your health and weight goals, taking into account any health conditions you currently have, or conditions you may be at risk for, and the best way to address them.
As you meet with your providers over the course of the year, you’ll go over how you’re feeling, where you are with your goals and what you need in terms of further support.
You’ll also receive weekly educational activities in your My Premise Health account, which take 3-5 minutes each to complete. Between visits, you’ll work on goals you created with your care team that support healthy lifestyle change.

If you’re new to Premise Health and haven’t had a visit with one of our providers, give your center a call to schedule your visit.
If you’re familiar with your Premise Health center and have visited before, you can schedule a visit by signing on to your My Premise Health account, selecting “Office Visit” and including “Beyond Weight” in the notes section.
Our program isn’t just for medications; it’s a comprehensive lifestyle program with specific eligibility requirements. In some instances, medication will be prescribed after three months of being in the program, but you cannot schedule an initial appointment with one of our providers simply to have weight loss medications prescribed or filled. Your Premise Health provider will determine if you are a good candidate for this program and for medications once you’ve actively enrolled.
Being successful starts with gaining a deeper understanding of who you are and what you value so that you know your “why.” Consider all the ways you see your life changing for the better when you’re at a healthy, sustainable weight. This might mean feeling more energetic, keeping up with your kids or grandkids, improving your overall health and more. Once you have your “why,” keep it in your mind and stay committed to the program and your goals. Build a trusting relationship with your provider where you’re honest about your challenges or setbacks. We are here to celebrate you through your victories and help you work through your difficulties.
While the cost to be on the Beyond Weight program can vary depending on your individual health plan, it is a cost-effective way to manage your weight. Typically, the cost is similar to your co-pay for a visit with your provider, with nurse visits being free. Additional costs will apply if you are prescribed medications like GLP-1s.
The only real difference between participating in the Beyond Weight program virtually versus in person is simply where you’ll meet with your providers. If you’re participating in person, you’ll go to your designated wellness center and meet with your providers there. With Virtual Beyond Weight, you’ll meet with a virtual care team. Regardless of which option you choose, you’ll get hands-on support, educational activities, compassionate guidance and personalized care.
To keep your care consistent, you’ll meet either virtually or in person throughout the entirety of the program.
While Beyond Weight is a 12-month program, your goals are the ultimate driver. You can end the program at 9 months or 24 months – it’s all dependent on where you are in your journey.
If you meet your goal before the 12 months are up, you can transition to the maintenance phase of your weight loss journey. You’ll continue to work with your care team to create a follow-up plan that works for you in the long term.
No, your employer will not have access to your health information.
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff.
Yes, monthly visits with your onsite care team can be scheduled either in person or by phone or video. If you’re already scheduled for an in-person visit and need to change to a virtual visit, we advise that you message your provider directly well in advance, or you may have to miss your appointment. If you’re enrolled in Virtual Beyond Weight, all your visits will be virtual.
If you miss an appointment, please reschedule as soon as possible. Missing two consecutive visits without a follow-up will result in being removed from the program.
If you were prescribed a GLP-1 weight loss medication prior to the program, your Premise Health provider will continue to monitor you on the dose you are on and adjust as appropriate.
Your prescriptions will be sent to your preferred pharmacy. Additionally, if your wellness center has an onsite pharmacy, you can make this your preferred pharmacy and save money.
