Who is Premise Health?
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When is the Eaton Health Center open?
Eaton Health Center
Monday – Thursday, 7:30 a.m. – 4:30 p.m.
Friday, 8 a.m. – 12 p.m.
The Health Center is located right inside the parking garage on the main floor.
Eaton Fitness Center
Monday – Thursday 6 a.m. – 7 p.m.
Friday 6 a.m. – 2 p.m.
Eaton Fitness Center is located past the Health Center at the end of Wellness Blvd.
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How much does a visit cost?
Eligible member costs:
- $45 for Primary Care visits and $35 for Physical Therapy (UHC insurance)
- $15 for Primary Care and Physical Therapy (Surest insurance)
- Wellness Coaching services are free of charge
- Free Preventive Services: Annual Physical Exam, Travel Visits and Laboratory Services
Who is eligible to use the Eaton Health Center?
All Eaton employees and dependents ages 2 and up are eligible to use the Eaton Health Center.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
