How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Will my current medical provider have access to my health records if I receive care at one of your centers?
How much does a visit cost?
There may be a $10 copay for your visit. Preventative visits are free (Physicals, Flu shots, and COVID-19 vaccines). There is a $10 charge for each prescription dispensed for medications that are not already free of charge through Cigna. *The clinic does not accept cash payments.
Who is eligible to use the centers?
Who is eligible to use The Summit Wellness Center?
The Summit Wellness Center is available to all regular, full-time associates of Hanesbrands who work at a HBI location in Forsyth County, NC. Spouses and Dependents (10 and up) who are enrolled in the company medical plan are also eligible to utilize The Summit Wellness Center.
Who is eligible to use The Health Center at Cherry Street?
The Health Center at Cherry Street is available to full-time HBI associates who physically work at the Cherry Street location.
Can I do lab/blood test orders from another provider at the center?
Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
What is the difference between preventive and non-preventive care visits?
Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.
Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
