Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues. 

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Mental health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Your provider will actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.

With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Provider-dispensed medications

Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand. 

The Summit Wellness Center carries close to 100 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling
  • STD testing and counseling
  • LGBTQ+ care
  • Education for self-awareness

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following onsite:  

  • Glucose or lipid testing 
  • Hemoglobin A1C
  • Rapid strep testing 
  • Flu testing 
  • Pregnancy testing 
  • Urine screenings 

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at The Summit Wellness Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community, including:
  • Influenza (Flu)
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Zoster (Shingles)

Wellness coaching

The Summit Wellness Center offers wellness coaching to help improve your overall wellbeing. Our certified wellness coaches will meet you where you are and help you make actionable goals to build new skills and make positive lifestyle changes. All coaching sessions are completely confidential and can take place virtually or in-person.

Our wellness coaches address a variety of lifestyle needs, including weight and stress management, nutrition coaching, physical activity routines, tobacco cessation, work/life balance and life satisfaction. They will help you reach your wellness goals through individual wellness coaching and goal setting, group education and lifestyle management programs, corporate challenges, and health and nutrition promotion through seminars, health fairs, meditations and podcasts. Wellness coaching is available in-person or virtually for all Hanesbrands associates and their eligible dependents within North Carolina. Virtual wellness coaching is available for Virginia associates.

Nutrition

Eating well can improve your overall wellbeing. It can also help treat existing conditions and decrease future risk. At The Summit Wellness Center, your registered dietitians use medical nutrition therapy to help treat a range of conditions, such as diabetes, hypertension and cardiovascular disease, as well as many digestive disorders and weight-related issues. They will tailor your nutrition counseling to meet you and your family’s unique needs and lifestyle with personalized action plans, based on your risk factors and family history.

Our registered dietitians can provide advice and ongoing education, in-person or virtually through one-on-one tailored sessions and individual nutrition assessments, group nutrition classes, lunch and learns, health fairs, seminars, podcasts, self-guided nutrition tools and virtual dietitian guided appointments to give you and your family a new appreciation for healthy food and, in turn, improve your overall health.

Travel medicine

At The Summit Wellness Center, we offer travel medicine services to help you and your family plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment or specialist referral.

My Premise Health

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

Get the same great care by phone or video.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Meet your care team.

Angela Brady-Fleming, Family Nurse Practitioner/Health Center Manager

Angela is a native of the Triad area and has worked in the corporate environment providing medical and wellness services to associates for more than 25 years. She has been with Premise Health since 2008 and at The Summit Wellness Center since its inception in 2010. In addition to primary care, Angela has experience in all types of nursing, including psychiatric and pediatric cancer, and time in the U.S. Army as a medic and a nurse. When not working, she enjoys house projects, church events, and spending time with her husband of 33 years, two adult children, her terrier mix Scruffy and granddog Shelby.

Heather Venoy, Registered Dietitian Nutritionist, Certified Diabetes Care and Education Specialist/Dietitian Coach-Wellness Program Manager

Heather is originally from West Virginia, moving to the Triad in 2017. Heather has 25+ years’ experience as a registered dietitian and 10+ years as a diabetes care specialist. She has a variety of experiences with many chronic disease conditions and wellness topics such as nutrition and physical activity. Heather has worked in both the inpatient hospital setting and outpatient clinics. When Heather is not working, she enjoys spending time with her family, with whom she is blessed with two amazing children and a loving, supportive husband of 25+ years. You will also find her enjoying time with her church family and, when time permits, curled up with a wonderful book. Heather joined the wellness team at The Summit Wellness Center in 2023 and is excited to work with each person to improve their health and wellness.

Olivia Smith, Medical Assistant

Olivia has been a medical assistant since 2012 and graduated from Carnegie Institute in Michigan. She moved to North Carolina in 2018 and joined The Summit Wellness Center in the spring of 2019. Olivia has worked in internal medicine, cardiology and pediatrics. She enjoys baking, thrifty shopping and taking care of her two Tamaskan dogs, Moose and Bear.

Premise Health is the organization that operates and staffs your wellness center. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

The Summit Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of nurse practitioners, care management registered nurses, medical assistants, registered dietitians and wellness coaches. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by Hanesbrands or their associates in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Eligibility and cost

The Summit Wellness Center is available to all regular, full-time associates of Hanesbrands who work at a HBI location in Forsyth County, NC. Spouses and Dependents (10 and up) who are enrolled in the company medical plan are also eligible to utilize The Summit Wellness Center.

There is no cost or out of pocket expense to be seen at The Summit Wellness Center. 

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need. 

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care, on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
