What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.
When is the Swagelok Health Clinic open?
Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How much does a visit cost?
Swagelok plan members can utilize the clinic for preventive visits at no cost. For Swagelok’s HDHP plan members, an office visit is $30 unless the annual deductible has been met. For Swagelok’s PPO plan members, an office visit is $10.
Lab services will be billed through the plan. The first fill of medications given by the Swagelok Health Clinic are included in the cost of your visit. Further prescriptions are to be filled by a community provider and will be billed through the plan.
Who is eligible to use the Swagelok Health Clinic?
Can my provider write me a prescription during a virtual visit?
A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed.
How do I schedule a virtual visit with my onsite provider?
Is a virtual visit appropriate for every medical condition?
Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need.
Is my health information secure? Does Swagelok have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
