How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When is the Scholastic Wellness Center open?
How much does a visit cost?
Copays, fees, deductibles, co-insurance and out-of-pocket costs depend on your plan. While some fees may apply, they will likely be lower than a traditional, community-based healthcare provider. Some services at the Wellness Center are provided at no cost to you. Please call the Wellness Center for further information.
Who is eligible to use the Scholastic Wellness Center?
Can I do lab/blood test orders from another provider at the center?
Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Is my health information secure? Does Scholastic have access to my personal health information?
Will my current medical provider or PCP have access to my health records if I receive care at the Scholastic Wellness Center?
What is the difference between preventive and non-preventive care visits?
Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.
Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
