At the Wellness Center, your health comes first. Team members like you get a comprehensive range of services to help you get, stay and be well, including primary care, lab services, mental health and more.

Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized care plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our care plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, obesity, diabetes and pre-diabetes, lower back pain and more. 

Care navigation

There may be times you need specialty care outside of our center. Finding that care on your own can sometimes be confusing. That’s why we offer care navigation services to support you in finding the affordable, high-quality care you need.

If your provider makes a specialist referral, our care navigation team will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your visits to help with any ongoing care needs.

Mental health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Our licensed counselors will actively work as part of your care team to support your overall health while making you feel safe and comfortable.

You can conveniently meet with them in person or virtually through video or by phone. With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

It’s easy to schedule a mental health appointment. Just sign on to My Premise Health, select “Make an Appointment” and choose “Behavioral Health.”

Pharmacy

Our pharmacy is a convenient way to fill your prescriptions. The pharmacists here are an extension of your care team, working with your provider as needed to support your overall health. We not only fill your prescriptions, we take time to get to know you so we can help you manage your conditions and reach your health goals.

We also offer a wide range of services that go beyond simply prescribing medications, such as personal health counseling, coaching and guidance, medication-specific education, as well as easy prescription transfers and low-cost over-the-counter medications.

Most members experience cost saving by transferring prescriptions to the Pharmacy.

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours, except for rapid tests, which are available during your appointment. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at the Wellness Center. Your provider or pharmacist can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer a variety of routine vaccines to protect your health and community, including:
  • Influenza (Flu)
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Varicella (Chickenpox)
  • Zoster (Shingles)

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

Get the same great care by phone or video.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

Premise Health is the organization that operates and staffs your wellness center and pharmacy. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

Pella Corporation Wellness Center and Pharmacy is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurses, medical assistants, care manager, care navigator, licensed mental health counselor and pharmacist. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by Pella or their team members in any way.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Eligibility and cost

All team members employed by Pella and dependents covered by a Pella medical plan (ages 2+) are eligible to use the center and pharmacy.

Costs can vary. Team members and covered dependents on non-HDHP Pella medical plans pay $0 for visits. Team members and covered dependents on a Pella HDHP plan pay $30 for visits and $0 after their deductible is met. Team members who are not enrolled on a Pella medical plan pay $30 for visits.

While you are not required to use the Wellness Center or Pharmacy, we hope you’ll consider trusting us with your overall wellbeing, much like Pella has trusted us as a partner in health. We’d love to see you soon!

All team members (including those team members that do not have insurance through Pella) are eligible to use the center and pharmacy. All dependents covered by a Pella medical plan (ages 2+) are also eligible to use the center and pharmacy.

Your personal health information is confidential, secure, and protected under federal law. Premise Health complies with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws to safeguard your information. Your health information will not be shared with Pella Corporation or any third party without your explicit written consent, except as permitted by law (for example, to respond to your request for a reasonable accommodation or as required for a work-related health injury). Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

The easiest way to make an appointment is through your online member portal. Visit MyPremiseHealth.com to set up your account and then schedule appointments online or through your My Premise Health mobile app.

You can also call (641) 200-1117 or visit during regular center hours to make an appointment.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

Virtual Primary Care is available to team members and dependents (ages 18+) enrolled in a Pella medical plan. Dependents on a Pella medical plan ages 2-17 can use Virtual Primary Care with a parent or guardian present.

If you have not done so already, first set up your account on the My Premise Health mobile app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will put you in touch with your provider to begin your visit.

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need.

You can call the Pharmacy at (641) 651-3400 to speak with a pharmacist.

You can easily drop off your prescriptions at Pella Corporation Pharmacy or your provider can e-prescribe them directly to the pharmacy. The providers and pharmacy at your wellness center act as one care team. Your provider will send your prescription to the pharmacist after your appointment so you can pick it up the same day, making for a seamless experience.

The Pella Pharmacy is conveniently located within the Wellness Center. This means you can get your prescriptions faster and easier. Also, our pharmacists go above and beyond what you experience at your typical pharmacy. They take the time to get to know you and work with your primary care provider to support your overall health.

Meet your care team.

Dr. Tim Dykstra, MD – Medical Director

Tim is a board-certified family medicine physician with over 25 years of experience. He graduated from Knoxville High School and Central College. After attending medical school at the University of South Dakota, he completed his family practice residency in Des Moines. He has worked in family medicine, urgent care medicine, occupational medicine, as well as emergency medicine in his career. He has lived in the Pella area for 18 years with his children all graduating from Pella High School. He is excited to practice in Pella again and believes Premise provides a climate that fosters collaboration and support, placing members’ wellbeing at the forefront. He believes that forming trusting relationships with his patients can make a positive impact on their healthcare. Outside of work, he likes to travel, exercise, and spend time with his family and friends.

Debra Northcutt, LMHC – Behavioral Health Provider

Debra is a licensed mental health counselor who earned a Master of Science in Family & Youth Services and a Master of Science in Clinical Counseling from Bellevue University. Raised in the Oskaloosa area, Debra spent 10 years in Omaha supporting at risk youth at Boys Town before returning to the area and settling in Pella with her family in 2014.

 

Debra brings experience from schools, community mental health clinics, correctional settings, and managed care, giving her a well-rounded and compassionate perspective on the challenges individuals and families face. She is passionate about recovery, resilience, and whole-person wellness, and strives to ensure every individual feels heard, respected, and supported.

Outside of work, Debra enjoys spending time outdoors, making music, and being with her family and pets. She continues to dream of new adventures, including songwriting, skydiving, cylcling. Known for her humor and encouragement, she values genuine connection and brings warmth and positivity to the workplace.

Jordan Ripley, BSN, RN – Health Center Manager

Jordan is the Health Center Manager and holds a Bachelor of Science in Nursing (BSN). She brings 11 years of nursing experience ranging from neonatal intensive care to nursing leadership and practice management. With a strong clinical foundation and leadership background, she oversees daily operations, supports the interdisciplinary team, and is committed to delivering efficient, patient-centered care in a compassionate, well-organized environment. Outside of work Jordan enjoys spending time outdoors with her husband and two children. Jordan’s family has a small hobby farm and spends much of the summer fishing for catfish.

Abby Kadera – Care Navigation

Abby is the Care Navigator at Premise Health. She has over 10 years of experience in a variety of different areas in healthcare including: Assisted living/Nursing home, EMS, ED, Volunteer Fire Department, and the clinic setting. Abby studied at DMACC in 2016 in the EMT program, and later went on to study at Mercy College of Health Sciences in 2018 as a part of the Paramedic program. Abby served as an EMT from 2016-2019 in both rural and large metro areas, with experience in both emergency response and interfacility transport. Abby enjoys talking with patients and takes pride in being a strong advocate to get them the care they need. On her time off, you can find Abby spending time with her husband and children outside, chicken/garden tending, or in various types of farm equipment during plant and harvest seasons.

Emma Pleima – Receptionist

Emma has over 5 years’ experience in the receptionist position and loves getting to be the welcoming smile you see when you walk in our door. With a background in multiple specialty settings, she is quick to provide the assistance needed. In her free time, you can find her playing with her nieces and nephews, snuggled up with a good book with a cup of coffee in hand, playing pickleball with her friends, or making a mess in the kitchen. Being a Pella native, she has a deep love for this town and getting to connect with the community in and outside of her role here at the Wellness Center.

Jen Poberzyn – Medical Assistant

Jen earned her Medical Assistant degree from Carrington College in Mesa, AZ, and soon after became a Registered Medical Assistant (RMA) through the American Medical Technologists (AMT). With over 20 years of clinical experience, she has worked in a wide range of healthcare settings—including internal medicine, family medicine, urgent care, home health, and occupational health. Drawing on her strong clinical and administrative expertise, she works closely with providers and staff to coordinate care, optimize workflows, and support a positive healthcare experience for every member. Jen is deeply passionate about the programs and services offered through Pella Corporation Wellness Center and is dedicated to fostering teamwork, clear communication, and compassionate care to achieve the best possible outcomes.

 

Outside of her professional life, Jen is a proud parent of two and cherishes spending time with her grown children, family, and friends. She enjoys traveling, attending a variety of festivals, fishing, and relaxing by the lake. When at home, Jen finds joy in cooking, gardening, and taking time to recharge while appreciating life’s simple pleasures.

Tashi Ethell, RN – Care Management

Tashi is an RN Care Manager who has worked as an RN since 2012. She completed her BSN through Mercy College of Health Sciences and her nursing experience includes helping members with their chronic conditions across many different types of health conditions and settings.


Tashi lives in the Knoxville area with her husband and 4 children. She enjoys crafts, reading, listening to podcasts/audiobooks, and all things true crime, but her favorite thing to do is attend her kids’ activities. Tashi is excited to be part of an amazing team providing accessible healthcare, education, and support to the Pella Corp employees and their families.

Ryan Collins, PharmD – Pharmacy Manager

Ryan Collins is the pharmacy manager of the Pella Corporation Pharmacy. Ryan graduated from South Dakota State University in 2023. He would like to use his experience and knowledge to improve your understanding, access, and costs of the medications you take. Jill Long, CPhT – Pharmacy Technician

Jill brings over 25 years of experience in the healthcare field in various roles including 10 years of pharmacy experience. Her experience includes hospital pharmacy, sterile compounding, and now retail pharmacy.
