Who is Premise Health?
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When is the Family Wellness Center open?
How much does a visit cost?
Services are available at no cost to all Nexteer employees and their family members enrolled in a Nexteer PPO/EPO medical plan. If you are on a High-Deductible Health Plan (HDHP), preventive care is available at no cost. For sick visits, those on a HDHP pay $25 per sick visit until your deductible is met, then $5 per visit after that.
Who is eligible to use the Family Wellness Center?
The Family Wellness Center is dedicated to Nexteer employees and their eligible dependents (ages 2 and up) who are enrolled in the Nexteer medical plan. The FWC is not open to the public.
Will my current medical provider or PCP have access to my health records if I receive care at the Family Wellness Center?
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Is my health information secure? Does Nexteer Automotive have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
