You have questions, we have answers. 

Premise Health is the organization that operates and staffs the health centers. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When you arrive at the gate:

  1. Press the intercom button and let security know you are there for a visit at MyHealth Center, they will open the gate.
  2. Or call MyHealth Center at 209-341-8930 and they will open the gate.

Get 24/7/365 access to an occupational health expert, wherever you are. If a work-related injury or illness happens, you can connect right away via My Premise Health or (855) 888-8878 for care, plus guidance on staying at work when appropriate and support for a safe return to work.

You also have phone and video access from anywhere in the U.S. to a team of virtual primary care providers around the clock. These providers will have access to your health records on My Premise Health and can easily communicate with your regular care team. With providers licensed in every state available 24/7/365, you can still get seen when on the go and during nights, weekends and holidays. In your My Premise Health account, select “Get care now” and we’ll connect you with the next available provider.

Yes, however, services may be limited.

After you have been treated at MyHealth Center, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Yes, the MyHealth Center can perform blood draws ordered by an outside provider, with the exception of Kaiser. We request all lab orders be faxed or dropped off at our office to ensure we have the proper supplies to run requested testing.

MyHealth Center is here to support you, and your family’s health needs, offering both onsite and virtual care options. If you and your dependents (ages 2 and older) are enrolled in a company-sponsored health plan, you have access to a wide range of care at MyHealth Center. For those participating in the Kaiser medical plan, you can take advantage of urgent care services—available onsite and virtually—for your convenience.

No matter which health plan you’re enrolled in, all employees can access Virtual Occupational Health services, specifically designed to help with work-related injuries and illnesses.

The MyHealth Center is an approved provider for workers’ compensation claims. Work-related injuries and illnesses will be treated onsite by a provider during regular business hours. Get 24/7/365 access to an occupational health expert, wherever you are. If a work-related injury or illness happens, you can connect right away via My Premise Health or (855) 888-8878 for care, plus guidance on staying at work when appropriate and support for a safe return to work.

Your personal health information is confidential and your data is kept secure. The MyHealth Center operates in accordance with HIPAA and works diligently to protect all health records. Premise Health cannot share any personal health information without your permission. Rest assured, your personal health information is not shared with your employer. Additionally, Premise Health operates through a private network and software systems that cannot be accessed by your employer. Notice of privacy practices are available for review within the center.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
