LP Health and Wellness Center offers a wide range of services to meet your unique needs. As your health ally, we focus on your overall wellness through preventive care, managing chronic conditions and more, rather than simply treating the symptoms that bring you in for a sick visit. Your providers will give you the convenient, quality care you need all in one place – in-person or virtually. They are also experts in coordinating care, which means they can refer you to specialists when needed and make sure nothing is overlooked.

Primary care

We offer a comprehensive range of primary care services that focus on your long-term health and preventive care. Your primary care provider will get to know you and your family, so they are better equipped to prevent, treat and manage any health issues that may come up. The best part? You can schedule same-day, next-day and virtual appointments and message your provider on your secure patient portal, My Premise Health. Remote member monitoring

LP Health and Wellness Center offers wellness coaching to focus on your overall wellbeing. Our certified wellness coaches will meet you where you are and help you make actionable goals to build new skills and make positive lifestyle changes. All coaching sessions are completely confidential and occur in-person at your health center.

Our wellness coaches address a variety of lifestyle needs, including weight and stress management, nutrition coaching, physical activity routines, tobacco cessation, work/life balance and life satisfaction. They will help you reach your wellness goals through individual wellness coaching and goal setting, group education and lifestyle management programs, corporate challenges, and health and nutrition promotion through seminars, health fairs, meditations and podcasts. Wellness coaching is available for all LP team members and their eligible dependents.

Prescription delivery

Skip the line at the pharmacy. Instead, get your prescriptions shipped right to your door at no extra cost. You’ll save money with low-cost generics, reduced co-pays and 90-day fills. You can also get discounted over-the-counter medications shipped with your prescription delivery.

When it’s time to refill, your provider will reach out to make sure you get your medications when you need them. If you have multiple prescriptions, they’ll even make sure to consolidate them into a single shipment. It’s personalized, convenient and saves you money. Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at LP Health and Wellness Center. Your provider can determine which ones are right for you and your family based on your age, conditions, travel habits and other factors. You can then easily schedule an appointment or simply walk in to get your vaccines.

We offer a variety of routine vaccines to protect your health and community, including Influenza and Tdap. For other vaccines, please reach out to your health center.

Lab services

To support you and your family’s needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following lab tests through the onsite wellness center only: glucose or lipid testing, hemoglobin A1C, rapid strep testing, influenza testing, urine pregnancy testing, urine dip screenings, rapid mono testing and H-pylori testing.
