Premise Health is the organization that operates and staffs your health center. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

  • $45 Primary Care visits and $35 for Physical Therapy (UHC insurance)
  • $15 for Primary Care and Physical Therapy (Surest insurance)
  • Wellness Coaching services are free of charge
  • Preventive services, including Annual Physical Exams, Travel Visits and Laboratory Services are free of charge
The Liberty Health Center offers confidential healthcare onsite and local virtual (State of MA and TX only) exclusively to Liberty Mutual benefit-eligible employees.
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
