What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Where is the JTEKT Employee Clinic located?
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.
When is the JTEKT Employee Clinic open?
Monday, Wednesday, 7 a.m. – 4 p.m.
Tuesday, Thursday, 8 a.m. – 5 p.m.
Friday, 6 a.m. – 12 p.m.
How much does a visit cost?
All services are free with no copays. If you are participating in JTEKT’s HSA insurance plan, those participants are required by the IRS to pay copays for acute need visits ($50 copay) and medications ($7 per medication).
Who is eligible to use the JTEKT Employee Clinic?
All JTEKT employees and dependents ages 2 years and up that are covered on a JTEKT health plan are eligible to utilize health center services.
Is my health information secure? Does JTEKT have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the JTEKT Employee Clinic.
Will my current medical provider or PCP have access to my health records if I receive care at JTEKT Employee Clinic?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Can the JTEKT Employee Clinic fill medication prescriptions from a community provider?
Yes, we can! Just schedule an appointment with a JTEKT Employee Clinic provider and bring your written prescription with you. If it is a medication we carry in our dispensary, we can fill the prescription with no copay. HSA participants will be required to pay a small copay for each prescription. Please note that we do NOT carry any narcotic medications onsite.
Can I do lab/blood test orders from another provider at the center?
Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
