Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your well-being, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Care management

You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues. 

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as high blood pressure, obesity, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Comprehensive family care
  • Contraception (birth control) counseling
  • STD testing and counseling

Physical therapy

You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your well-being. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.

We offer a full range of in-person and virtual services, including:

  • Therapeutic exercise and conditioning
  • Pre- and post-operation rehabilitation
  • Ergonomics evaluations
  • Orthopedic and neurological rehabilitation
  • Pediatric orthopedic
  • Health management and awareness education

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider. 

We can perform and process the following onsite:

  • COVID-19 testing
  • Glucose or lipid testing 
  • Hemoglobin A1C
  • Rapid strep testing 
  • Flu testing 
  • Pregnancy testing 
  • Urine screenings 
  • Rapid mono testing

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at the Humana Health & Well-being Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors. 

We offer a variety of routine vaccines to protect your health and community, including:

  • Influenza (Flu)
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Varicella (Chickenpox)
  • Zoster (Shingles)

Travel medicine

At the Humana Health & Well-being Center, we offer travel medicine services to help you and your family plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment, or specialist referral.

Care navigation

There may be times you need specialty care outside of our center. Finding that care on your own can sometimes be confusing. That’s why we offer care navigation services to support you in finding the affordable, high-quality care you need.

If your provider makes a specialist referral, our care navigation team will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your visits to help with any ongoing care needs.

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

Get the same great care by phone or video.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same cost as an in-person visit.

  • Follow-up appointment or to review test results
  • Cough, fever or sore throat
  • Nausea, vomiting or diarrhea
  • Cold, flu, COVID-19 or allergy symptoms

24 hours a day, 7 days a week, 365 days a year

You also have access to a virtual care team for your primary care needs after hours. Sign in to My Premise Health and select “Get Care Now” to be seen right away.

Premise Health is the organization that operates and staffs your health and well-being centers. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

The Humana Health & Well-being Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, care management registered nurses, physical therapists and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your health and well-being centers are not staffed or managed by Humana or their associates in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Services are available to Humana associates located in or visiting Louisville and family members (newborn and above) on the company medical plan. 

  • No cost for preventive care services.
  • $30 Provider visit for primary care, urgent care and chronic condition management.
  • $45 Physical therapy visit / $30 Virtual physical therapy visit.

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the Humana Health and Well-being Center.

Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

In most cases, same-day appointments are available. Schedule online or call your health and well-being center to see if a same-day appointment is available.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time. Same-day appointments are typically available.

Virtual visits

If you have not done so already, first set up your account on the My Premise Health mobile app or online at mypremisehealth.com. Then, log in and select “Get Care Now.” A Premise Health team member will put you in touch with your provider to begin your visit.

To meet virtually with one of the providers at your health and well-being center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need. 

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your well-being. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your health and well-being center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

Dr. Jennifer Craig, MD
Physican

Meet Jennifer, our board-certified family physician. Jennifer has been practicing primary care for 21 years providing care for infants through geriatrics. She has been actively involved with a local hospital, sitting on multiple committees over the years including the ethics committee, infection control, quality utilization and review, morbidity and mortality, the medical executive committee and has served in various leadership positions including medical director of a rural health clinic.

Jennifer is involved in her community and serves on the Children’s Fatality Review Committee to assist in forming policies to help with prevention, previously served on the board of the Department of Children’s Services and is actively involved on a board of an addiction recovery ministry. She enjoys the diversity of family practice and the opportunity to foster long-term relationships with her patients which leads to a better opportunity to affect positive changes to their health. Jennifer also enjoys working collaboratively with her team.

Kristin Fuller, PA-C
Physician Assistant

Meet Kristin, our physician assistant. Kristin grew up in Ohio and attended Baldwin-Wallace College for her undergraduate degree in exercise physiology. She became a certified medical assistant and worked in radiology at a hospital prior to graduate school. Kristin attended Shenandoah University in Virginia for Physician Assistant School and graduated in 2010. She has been working in family practice since graduation and is passionate about helping others live a healthy lifestyle, diabetes management and preventative care. Kristin is NCCPA certified and a member of the American Academy of Physician Assistants.

Rob Francis, APRN 
Nurse Practitioner

Meet Rob, our board-certified FNP who earned his MSN at Bellarmine University. He started his nursing career in the ICU, then served as an advanced practice RN for patients with complex medical conditions. In 2017 Rob transitioned to the Humana care team. He believes that member-provider relationships should allow shared decisions about health and well-being and strives to empower members to be proactive while developing a trusting relationship with Premise Health providers.

Jessie Konkler, RN, MSN
Health Center Manager

Meet Jessie Konkler, our health center manager with a heart for wellness and family. Jessie earned her Bachelor and Master degrees in nursing from Indiana University Southeast. Since 2013, Jessie has been on the frontlines of healthcare, bringing her expertise to various fields such as labor and delivery, nursery care, medical-surgical units, hospice care and leadership roles. Jessie calls southern Indiana home, where she enjoys life’s adventures with her husband, four sons and four dogs. A lover of the outdoors, Jessie finds joy in camping, swimming, raising German Shepherds and exploring the countryside on side-by-sides. Above all, Jessie is committed to guiding members toward achieving and maintaining their best health.

Referral Coordinator

Meet Erin, our care navigator who has been a medical assistant for 30 years. She has worked in many areas including pediatrics, internal medicine, urgent care, general surgery and her current role as a care navigator. She has been with Premise Health for three years. Erin is a native Minnesotan, where three seasons there are beautiful! She has four children and 12 grandchildren. She loves spending time with family and friends, gardening, her puppy Sebastian (Bash) and cheeseburgers.

Carry Doyle, MSN, RN
Care Manager

Meet Carry, our care manager. Carry has her Bachelor’s in nursing from Eastern Kentucky University and a Master degree in nursing education from Gonzaga University. She has experience in medical/surgical nursing, critical care, open heart recovery, nursing education, diabetes education and most recently worked as a clinical research nurse. As a former certified diabetes educator and a diabetes prevention program lifestyle coach, Carry has a wide variety of teaching experience in both the classroom setting and in a one-on-one approach. She likes to engage each member as an individual with specific goals targeted to their unique needs. Carry believes that individualization is key to successful member involvement and motivation to help reach healthy goals and improve quality of life.
