Physical therapy

You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your overall well-being. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications so you can get back to moving with ease.

We offer a full range of services for you and your family, in-person or virtually, that include therapeutic exercise and conditioning, pre- and post-operation rehabilitation, ergonomics evaluations onsite or virtually, orthopedic and neurological rehabilitation, pediatric orthopedic, health management and awareness education. 

Travel medicine

At the Humana Health & Well-being Center, we offer travel medicine services to help you and your family plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment, or specialist referral.

Women's health

Women have unique healthcare needs. Our providers spend time getting to know you, or the women in your life, to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health.

Along with general primary care services, we offer the following specifically for women’s health: breast cancer screenings, pregnancy testing, pelvic exams, contraception counseling, STD testing and counseling, cervical cancer screenings (Pap smears), counseling for interpersonal and domestic violence, LGBTQ+ care, comprehensive family care, hormone therapy and education for self-awareness. Women’s health services are available for all Humana associates and covered family members.

Care navigation

The Humana Health & Well-being Center offers many services to cover your health needs. However, there may be times you need specialty care outside the center. Finding that care on your own can sometimes be confusing. That’s why we provide care navigation to support you in finding the affordable, high-quality care you need.

If your provider gives you a specialist referral, your care navigator will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your specialist visit to help with any ongoing care needs.

If you live in the Louisville area, you also have direct access to Healthcare Bluebook, our care navigation tool, online or through the mobile app. It allows you to search for care and save money on common health services and procedures. Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at Humana Health & Well-being Center. Your provider can determine which ones are right for you and your family based on your age, conditions, travel habits and other factors. You can then easily schedule an appointment or simply walk in to get your vaccines.

We offer a variety of routine vaccines to protect your health and our community, including Influenza, Hepatitis A, Hepatitis B, Human Papilloma Virus (HPV), MMR, Pneumococcal, Tdap/Td, Varicella and Zoster.

Lab services

To support you and your family’s needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following lab tests onsite: glucose or lipid testing, hemoglobin A1C, rapid strep testing, influenza testing, urine pregnancy testing, urine dip screenings and rapid mono testing. We also offer COVID-19 testing. 

*Turnaround time for results is subject to national trends and case surges.

Care management

Care management offers guided support when you need it most. Whether you’re juggling multiple conditions or just need a little extra support getting your health in check, your care manager is here to help.

You have a dedicated care manager available through your wellness center. As a registered nurse, they’ll work with your care team to provide a personalized care plan with ongoing support as well as coaching and education to address health concerns that are impacting your life. Our Care Managers can help you get to the root cause of your health issues and assist you in making informed decisions that could lead to some positive changes. Through hands-on guidance and regular check-ins, they can help with a range of health concerns and conditions, including diabetes, cancer, Lupus, hypertension, mental health issues or any other health challenge or condition that may require a need for frequent touchpoints with your care team. Schedule an appointment with your provider and ask about our care management program.

It’s easier than ever to get the care you need.

You can get the same great care you receive at your well-being center, virtually, either online or over the phone. This safe and convenient option gives you the flexibility to schedule a virtual appointment with your trusted provider during your center’s regular business hours, at the same cost as an in-person visit. Get the care you need at home, in the office or on the go.

You also have the benefit of virtual primary care. This benefit gives you 24/7 access to on-demand primary care with your trusted provider the moment you or your family need(s) them. No appointment needed. Learn more about our virtual offerings.

Jessie Konkler, RN, MSN
Health Center Manager

Meet Jessie Konkler, our Health Center Manager with a heart for wellness and family. Jessie earned her Bachelor’s and Master’s degrees in Nursing from Indiana University Southeast. Since 2013, Jessie has been on the front lines of healthcare, bringing her expertise to various fields such as labor and delivery, nursery care, medical-surgical units, hospice care, and leadership roles. Jessie calls Southern Indiana home, where she enjoys life’s adventures with her husband, four sons, and four dogs. A lover of the outdoors, Jessie finds joy in camping, swimming, raising German Shepherds, and exploring the countryside on side-by-sides. Above all, Jessie is committed to guiding our members toward achieving and maintaining their best health.

Dr. Jennifer Craig, MD
Physican

Meet Dr. Craig, our board-certified family physician. Dr. Craig has been practicing primary care for 21 years providing care for infants through geriatrics. She has been actively involved with a local hospital, sitting on multiple committees over the years including the ethics committee, infection control, quality utilization and review, morbidity and mortality, the medical executive committee and has served in various leadership positions including medical director of a rural health clinic.

Dr. Craig is involved in her community and serves on the Children’s Fatality Review Committee to assist in forming policies to help with prevention, previously served on the board of the Department of Children’s Services and is actively involved on a board of an addiction recovery ministry. She enjoys the diversity of family practice and the opportunity to foster long-term relationships with her patients which leads to a better opportunity to affect positive changes to their health. Dr. Craig also enjoys working collaboratively with her team.

Rob Francis, APRN 
Nurse Practitioner

Meet Rob, our board-certified FNP who earned his MSN at Bellarmine University. He started his nursing career in the ICU then served as an advanced practice RN for patients with complex medical conditions. In 2017 Rob transitioned to the Humana care team. He believes that patient-provider relationships should allow shared decisions about health & well-being & strives to empower patients to be proactive while developing a trusting relationship with Premise Health providers.

Kristin Fuller, PA-C
Physician Assistant

Meet Kristin, our physician assistant. Kristin grew up in Ohio and attended Baldwin-Wallace College for her undergraduate degree in Exercise Physiology. She became a certified medical assistant and worked in radiology at a hospital prior to graduate school. Kristin attended Shenandoah University in Virginia for Physician Assistant School and graduated in 2010. She has been working in family practice since graduation and is passionate about helping others live a healthy lifestyle, diabetes management and preventative care. Kristin is NCCPA certified and a member of the American Academy of Physician Assistants.

Referral Coordinator

Meet Erin Komoto, our Care Navigator who has been a Medical Assistant for 30 years. She has worked in many areas including pediatrics, internal medicine, urgent care, general surgery, and her current role as a Care Navigator. She has been with Premise Health for three years. Erin is a native Minnesotan, where three seasons there are beautiful! She has four children and 12 grandchildren. She loves spending time with family and friends, gardening, her puppy, Sebastian (Bash) and cheeseburgers.

Carry Doyle, MSN, RN
Care Manager

Meet Carry, our Care Manager. Carry has her Bachelor’s in Nursing from Eastern Kentucky University and a Master’s in Nursing Education from Gonzaga University. She has experience in Medical/Surgical Nursing, Critical Care, Open Heart Recovery, Nursing Education, Diabetes Education, and most recently worked as a Clinical Research Nurse. As a former Certified Diabetes Educator and a Diabetes Prevention Program Lifestyle Coach, Carry has a wide variety of teaching experience in both the classroom setting and in a one-on-one approach. She likes to engage each patient as an individual with specific goals targeted to their unique needs. Carry believes that individualization is key to successful patient involvement and motivation to help reach healthy goals and improve quality of life.

Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

The Humana Health and Well-being Center is staffed, managed and operated by Premise Health. Your medical staff includes a combination of physicians, nurses, physical therapists, and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. The Humana Health & Well-being Center is not staffed or managed by Humana or their associates in any way.

Yes. Services are available to Humana associates located in or visiting Louisville and family members (newborn and above) on the company medical plan.

• No cost for preventive care services.
• $30 Provider visit for primary care, urgent care and chronic condition management.
• $45 Physical therapy visit / $30 Virtual physical therapy visit. 

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the Humana Health & Well-being Center.

The easiest way to make an appointment is through your online member portal. Visit mypremisehealth.com to set up your account and then schedule appointments online or through your My Premise Health mobile app.

While walk-ins are gladly accepted, the preferred method is to schedule an appointment ahead of time.
