How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When are the Wellness Center, Pharmacy and Fitness Center open?
Wellness Center:
Monday – Thursday, 7 a.m. – 6 p.m.
Friday, 7 a.m. – 5 p.m.
Second Saturday each month, 8 a.m. – 12 p.m.
Physical Therapy services are available Monday – Friday, 7 a.m. – 5:30 p.m.
Pharmacy: Monday – Friday, 9 a.m. – 5 p.m.
Fitness Center: 24/7
How do I contact the Wellness Center and Pharmacy?
How much does a visit cost?
Preventive care services, including annual physicals, labs and vaccines, are generally free. However, a $25 charge applies if a diagnosis is made at your visit. Most routine visits, such as office visits and physical therapy sessions, cost $25. Follow-up care, like injections and care management visits, have no charge.
Additional services vary: low-cost labs are $12; some higher-cost items include orthotics ($110). Virtual primary and mental health care are $0.
Who can use the Greenheck Wellness Center?
Greenheck Group Wellness Center, Pharmacy and Virtual Care services are operated by Premise Health and are available to all team members, spouses and dependents enrolled in a Greenheck Group medical plan.
Who can use the Fitness Center?
The Fitness Center is available to Greenheck Group team members and their spouses, regardless of medical coverage. Dependent children are not eligible. An approved membership form is required for badge access. The form can be found here.
How can I become a member of the Fitness Center?
Complete and submit the Fitness Center Membership Form online.
Can my medical records be shared between Premise Health and my regular primary care provider (PCP)?
You can request that your PCP share your records with Premise Health at the Greenheck Group Wellness Center by completing your provider’s record-sharing process. Additionally, you may request that Premise Health share your records with your PCP or another specialist.
NOTE: All services at the Greenheck Group Wellness Center are confidential and protected under federal HIPAA privacy laws, which ensure your health information remains private. Only you and your healthcare provider can access your medical information, unless you give explicit permission otherwise.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Does Greenheck Group or my employer have access or see my personal medical information at the Greenheck Group Wellness Center?
No. Greenheck Group/your employer cannot access or view your personal medical information. All services at the Greenheck Group Wellness Center are strictly confidential and protected under federal HIPAA privacy laws, which ensure your health information remains private. Only you and your healthcare provider can access your medical information, unless you give explicit permission otherwise.
What type of medical staff will work at the wellness center?
Premise Health has recruited a high-quality team of dedicated healthcare professionals to staff and manage your wellness center. The team includes a nurse practitioner to lead your care, a nurse or medical assistant for routine support and a registered nurse who manages the center. You’ll also have access to a care manager, a physical therapist and a pharmacist — all backed by physician oversight.
