Whether you’re at home, at work or on the go, you and your family deserve convenient access to quality care. That’s why Premise Health provides you with a few simple ways to see a provider by phone or video [at no cost to you].

Here are the virtual options available to you:

Phone or video appointments with your trusted provider during your center’s regular business hours

Best for: Those who want to see their local provider without traveling to the wellness center.

On-demand, 24/7 access to care anytime, anywhere with a provider from Premise Health’s virtual care team

Best for: When you [or a family member] need to see a provider after hours or while traveling away from home.

Visits with a designated virtual primary care provider (PCP) for your ongoing needs

Best for: Members who prefer to receive care in a virtual setting with the same dedicated provider for every visit. Learn more here or call (833) 930-4530.

How to get started

You can talk to a provider by video or phone on the My Premise Health app or at mypremisehealth.com.

To schedule a virtual visit with a provider from your wellness center, log in to your My Premise Health account and select “Schedule an Appointment.” Choose your desired appointment type from the available “Telephonic” or “Video” options and complete the prompts.

If you need care right away, log in to My Premise Health, select “Get Care Now” and follow the remaining steps to request an on-demand visit.

What comes next?

For scheduled virtual appointments with your local provider:

Please complete the eCheck-in at least 10 minutes before your visit in My Premise Health. Your provider will then join the video chat, or call if you selected a telephonic visit, at your appointment time.

For on-demand care:

After you complete the prompts in My Premise Health, you will receive a call from Premise Health Member Services, who will connect you with the next available provider.
