Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you and your family so they are better equipped to prevent, treat and manage any health issues that may come up. 

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Women's health

We understand a woman’s healthcare needs are unique. Your provider will spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health. 

Along with general primary care services, we offer the following specifically for women’s health:

  • Breast cancer screenings
  • Pelvic exams
  • Cervical cancer screenings (Pap smears)
  • Pregnancy testing
  • Contraception (birth control) counseling
  • STD testing and counseling

Lab services

We offer many basic lab services to support you and your family’s needs. We can perform onsite testing and can also collect and send samples to an offsite lab. 

You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider. 

Wellness

Our wellness programs promote nutrition, physical activity, stress management, healthy relationships and more. We take a comprehensive approach that focuses on your overall health and wellbeing. Our providers will personalize your wellness plan and help you make positive lifestyle changes that can lead to a more balanced and healthier you.

Your wellness services include biometric screenings, flu vaccinations, vaccines, annual physicals, sports physicals for school-age children, women and men’s health exams.

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.

You can schedule a virtual appointment with your trusted care team during our regular business hours, at the same no cost as an in-person visit.

Premise Health is the organization that operates and staffs your wellness center. We are the nation’s leading provider of direct healthcare, which means we partner with companies to offer their employees high-quality in-person and virtual health services. We are here as your health ally to help you get, stay and be well. We do this with experience, and by shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

Members are what we call our patients. If your employer has partnered with Premise Health to provide access to our wellness centers and services, you’re automatically considered an eligible member. There is no membership fee to get started. It also reflects how much we value our care relationship with you, because to us, you’re more than just a patient – you’re a member.

The City of Mesa Health and Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of providers, allied health professionals, administrative assistants and a health center manager. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness centers is not staffed or managed by the City of Mesa or their employees in any way. 

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Eligibility and cost

City of Mesa employees, their dependents, or retirees that are on the City of Mesa health plan (Cigna), are eligible to utilize the health center services.

Utilizing the City of Mesa Health and Wellness Center operated by Premise Health is cost effective. All services are at no charge. Lab, referral providers/services, and prescriptions are subject to insurance.

Confidentiality

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.

Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Scheduling

You can easily schedule appointments through the My Premise Health mobile app or online at mypremisehealth.com. Once you create your account, sign in and select “Schedule an Appointment” and choose your appointment type.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

Yes. Please schedule an appointment ahead of time via My Premise Health or phone call. Some same-day appointments are typically available for acute care needs.

Virtual visits

To meet virtually with one of the providers at your wellness center, you must first create an account on the My Premise Health app or online at mypremisehealth.com. Log in and select “Schedule an appointment,” choose your appointment type and if prompted, select whether you’d like to be seen by phone or video. You can then follow the remaining prompts to complete your scheduling.

A virtual visit is just like an in-person appointment. You can discuss your medical needs with your provider, and they will diagnose and prescribe medications as needed. 

Virtual visits are designed to treat your short-term needs and provide long-term preventive care with convenience and flexibility. If you’re experiencing a medical emergency, please dial 911 as soon as possible to get the medical attention you need. 

Services

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits also known as acute visits and treatment for issues like cold and flu, allergies, injuries and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

Dr. Adrian Pinzon- Medical Director- Adult and Pediatric

Dr. Adrian Pinzon, a highly accomplished medical professional with nearly two decades of experience, has demonstrated exemplary leadership and dedication in family medicine. Currently serving as Medical Director at Premise Health in Mesa, Arizona, Dr. Pinzon has also held similar leadership roles at OnSite Care Clinics and My Dr Now. A graduate of St. Christopher’s College of Medicine and the Racine Family Medicine Residency Program, where he served as Chief Resident, Dr. Pinzon combines extensive clinical expertise with a passion for preventive care, having organized community health initiatives like diabetes screenings. Fluent in English and Spanish, he is an ABFM Board-Certified Family Physician and an active member of the American Academy of Family Physicians. Known for his commitment to patient care and innovative healthcare solutions, Dr. Pinzon brings a wealth of knowledge and a collaborative spirit to every role he undertakes.

Kaylen Ballenger NP- Adult and Pediatric

Kaylen was born in Utah but has spent the majority of her life in Mesa, Arizona. She graduated from Grand Canyon University in 2013 with her Bachelor of Science in Nursing and worked as a Registered Nurse in the inpatient setting for 8 years before completing her Masters of Science in Nursing, also from Grand Canyon University, in 2021. Prior to coming to the City of Mesa Health and Wellness Center Kaylen worked as a Nurse Practitioner in Cardiology. While she enjoyed working in the cardiology field, her heart was and continues to be in primary care. In her free time, Kaylen enjoys spending time with her spouse and her two young children and is particularly fond of boating!

Dustin Ivanic NP- Adult and Pediatric

Dustin Ivanic is a board-certified Family Nurse Practitioner with over 30 years’ experience in medicine. He completed his undergraduate education at Arizona State University and postgraduate education at Grand Canyon University. His medical background includes EMS, sports medicine, cardiology, and chronic disease management. He has strong clinical experience managing patients with cardiovascular disease, diabetes, obesity, and orthopedic conditions. He is a proud proponent of preventative health care. He has been married to his wife Francesca for 28 years and they have two adult children. When away from work, Dustin enjoys traveling, watching NFL football, and reading.

Angela Gorazd NP- Adult/ Women’s Health

Angela Gorazd is a board-certified Adult Nurse Practitioner with over 25 years’ experience in the medical field.   She completed her undergraduate education at Southern Illinois University Edwardsville, and her post graduate MSN at Goldfarb School of Nursing at Barnes Jewish College, and her Doctorate at Maryville University in St. Louis, MO. Her medical background includes primary care, internal medicine, women’s health, and post-acute rehabilitation.  Angela takes pride in educating her clients about the importance of preventative care.  She recently moved to Arizona from Illinois. She has two adult sons.  Angela enjoys gardening, reading, hiking, bicycling, and exploring new adventures.  She believes being active physically and mentally is the key to good health.

Michelle Vidaure NP- Adult and Pediatric

Michelle Vidaure was born and raised here in Arizona. She graduated from Corona Del Sol and went to nursing school at Gateway Community College. Her career started 17 years ago as a Registered Nurse. She worked in Med/Surg and telemetry at different hospitals around the valley and  completed her master’s degree at Grand Canyon University. She has been a Nurse Practitioner for 10 years with a specialty in Family Practice working with the City of Mesa team since 2019. When she is not at work you can find her spending time with her Frenchie (Mya), reading a book or watching a movie.

Kiran Dhillon NP- Skin Cancer screenings only- call for an appointment

Kiran is a Dermatology Nurse Practitioner originally from Winston-Salem, North Carolina. She earned both her Bachelor of Science in Nursing (BSN) and Master of Science in Nursing (MSN) from Emory University in Atlanta, GA, graduating summa cum laude. After relocating to Arizona four years ago, she has been practicing dermatology, specializing in both medical and cosmetic dermatology. Outside of her professional work, Kiran enjoys hiking, running, and spending time with her husband, Jay, and her dog, Rio.
