Your primary care provider will get to know you and your family, so they are better equipped to prevent, treat and manage any health issues that may come up.
Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.
You don’t have to navigate your health challenges alone. With our care management services, you and your family can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain, and more. We can provide one-on-one coaching, disease-specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling, and preventive and ongoing monitoring to manage your symptoms and help you and your family stay well.
City of Kingsport Diabetes Program:
The City of Kingsport Employee Wellness Center has a Diabetes program to help you better manage your condition. Participation in the program may be able to provide you with free testing supplies and medications. Schedule an appointment with one of your Premise Health providers for more information on this program.

We offer many lab services to support you and your family’s needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

Your wellness services include biometric screenings, wellness coaching, medication consultations, and condition management.
Wellness coaching can help you achieve lasting lifestyle change in the areas of weight management, nutrition, physical activity, sleep, stress, smoking cessation, work/life balance and life satisfaction. Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and can take place virtually or in person.
Keeping you safe at work is a top priority, which is why we offer a full suite of occupational health services. Your providers at The City of Kingsport Employee Wellness Center are here to treat work-related injuries and illnesses to help you get back to feeling well.
Our occupational health services are available for all City of Kingsport employees and include injury diagnosis and treatment, musculoskeletal evaluations, specialist referrals when needed, and return-to-work plans.

Skip the extra trip to the pharmacy. You can get your medications at no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
The City of Kingsport Employee Wellness Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.

My Premise Health is your member portal. It is a secure online and mobile tool that you can use to manage your health information. My Premise Health connects you to your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
Yes, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

In most cases, same-day appointments are available. Schedule online or call your wellness center to see if a same-day appointment is available.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
Note: not all labs can be drawn from the health center. You will be able to discuss available options with the nurse.
Yes, you can! Just schedule an appointment with a provider in the wellness center to discuss and bring your written prescription or medication bottle(s) with you to your appointment.

The City of Kingsport Employee Wellness Center has a program just for you! Participation in the program may be able to provide you with free testing supplies and medications. While the City of Kingsport Employee Wellness Center does not provide full pediatric services, we can provide acute pediatric care for children ages 2 years and up, which can include care for colds and flu, and school and sports physicals.
