How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

When is the Frisco Employee Wellness Center open?
The hours of operation are:
Monday, Wednesday, Friday, 8 a.m. – 5 p.m.
Tuesday & Thursday, 6:30 a.m. – 3:30 p.m.
Closed for lunch, 12 – 1 p.m.
Is my health information secure? Does the City of Frisco have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We cannot and do not share any personal health information with your employer or anyone else without your permission, unless it pertains to a work-related health injury or job-related return-to-work status. Written consent will be obtained from you prior to that disclosure. Premise Health also operates through a private network and software systems that cannot be accessed by other patients or City of Frisco employees. You can review a notice of privacy practices within the Frisco Employee Wellness Center.
How much does a visit cost?
Your provider visit, onsite and send-out labs are all covered at no cost to eligible employees and dependents.
Who is eligible to use the Frisco Employee Wellness Center?
All full-time employees and dependents ages 2 and up who are enrolled in the wellness benefit or the City of Frisco health insurance plan, and all part-time employees, are eligible to use the Wellness Center.
Will my current medical provider have access to my health records if I receive care at the Frisco Employee Wellness Center?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
Please call the Frisco Employee Wellness Center at (469) 604-0800 or send a message via My Premise Health to confirm the easiest way to request your records be sent to your provider.
How do I contact the Frisco Employee Wellness Center regarding my appointment?
To schedule, reschedule, or cancel an appointment, you may call the Frisco Employee Wellness Center at (469) 604-0800, stop by the center during its regular business hours, log in to mypremisehealth.com or use the My Premise Health app.
If you are unable to keep your scheduled appointment, please cancel or reschedule so other patients who need an appointment can schedule for that time.
Can My Premise Health be used to schedule work-related physicals or for work-related injuries?
No. My Premise Health cannot be used to make appointments for your work-related physicals (Police and Fire) or for work-related injuries.
Appointments for Occupational Health visits must be made by calling the Frisco Employee Wellness Center at (469) 604-0800.
What should I do if I get injured at work?
Please notify your Supervisor as soon as the injury occurs. After reporting it, call the Frisco Employee Wellness Center at (469) 604-0800 to schedule an appointment so our team can evaluate your injury and provide the appropriate care.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
What are the differences between preventive and non-preventive visits?
Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, well-woman exams, cancer screenings, some types of blood work, vaccinations and more.
Non-preventive care includes scheduled visits when you are not feeling well or for chronic disease management such as high blood pressure, diabetes, thyroid disorders, obesity and others. These visits are meant to address your symptoms, such as cold and flu, sore throat, allergies, injuries and more. In addition, these visits are for regular follow-up with a provider and can include blood work for patients with chronic disease to help you get, be and stay well.
Can the Frisco Employee Wellness Center be my Primary Care Provider (PCP)?
Yes, the Frisco Employee Wellness Center can serve as your Primary Care Provider. Choosing us as your PCP helps ensure consistent, coordinated care and makes it easier for our team to support your long‑term health. We also recommend regular follow‑up visits so we can stay ahead of any health concerns and keep you feeling your best.
