- Treatment and management of conditions
- Preventive screenings for early disease detection
- Biometric screenings
- Vaccinations
- Allergy and flu shots
- Minor illness and injury visits
- Pediatric care (ages 2+)
- And more
Care management
You don’t have to navigate your health challenges alone. With our care management services, you can get the support you need. This no-cost benefit provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, obesity, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling and ongoing monitoring to manage your symptoms and help you stay well.
Provider-dispensed medications
Skip the extra trip to the pharmacy. You can get your medications at little to no cost directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
The City of Bristol Health and Wellness Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.
