Premise Health is the organization that operates and staffs the health centers. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

The hours of operation for the AZ Employee Health and Wellness Centers are listed here.

The AZ Employee Health and Wellness Center is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, care management registered nurses, physical therapists and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by AstraZeneca or their employees in any way.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
If the AZ employee has the HDHP, there is a $25.00 encounter fee until deductible is met, then $10.00 until both deductible and Out of Pocket is met.

All AstraZeneca employees can visit the AZ Employee Health and Wellness Center.

In most cases, same-day appointments are available. Walk in or call your wellness center to see if a same-day appointment is available.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the wellness center.
Yes, your health and wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.
