Premise Health is the organization that operates and staffs your wellness center. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Oakland, CA:
Monday – Friday: 7:30 a.m. – 4:30 p.m. PT (Lunch: 12:30 p.m. – 1:30 p.m. PT)

All primary care visits are available at no cost to you. Other services such as Acupuncture, Chiropractic, Physical Therapy, Care Management, and Wellness Coaching are available for $25 per visit.

All Premise Health employees, spouses, and dependents are eligible to use the Premise Health Wellness Centers.

After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
