We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Cost for services vary based on type of services that are being rendered as well as which medical plan a member is enrolled in. Anthem PPO and EPO are responsible for a co-pay and Anthem PPO Saver is responsible for unmet deductible and co-insurance until OOPM is met. Most services offered in the onsite health center are covered by Kite at no out-of-pocket cost to our members.

All members who are employed by Kite Pharmaceuticals are eligible.
