Who is Premise Health?
How do I make an appointment?
You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

How much does a visit cost?
- Acute care visits are at no cost. Insurance will be applied for primary care follow up visits.
- If you are enrolled in the UnitedHealthcare Consumer Plan or either of the Choice Fund Open Access Plus HSA Plans (HDHP 1600 or HDHP 3000), a primary care visit costs $35 toward your deductible. Once you reach your deductible, visits are free.
- If you are enrolled in the UnitedHealthcare Classic Plan, a primary care visit costs $30 per visit.
- If you are enrolled in the Open Access Plus Plan (OAP 600), a primary care visit costs $10 per visit.
- If you are enrolled in Surest plan, a primary care visit costs $10 per visit.
- If you are eligible for medical coverage through UnitedHealthcare or Cigna but waived it, primary care visits cost $50.
Who is eligible to use the Hearst Wellness Center?
All other employees can access acute care at the Hearst Wellness Center, including immunizations and vaccinations and treatment for common conditions such as colds, flu, infections, allergies, and more, but they are not eligible for primary care services.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Is my health information secure? Does Hearst have access to my personal health information?
Will my current medical provider or PCP have access to my health records if I receive care at the Hearst Wellness Center?
What is the difference between preventive and non-preventive care visits?
Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.
Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
Acute care visits are at no cost. Insurance will be applied for primary care follow up visits.
