Primary care

We offer a comprehensive range of primary care services that focus on your long-term health and preventive care. Your primary care provider will get to know you, so they’re better equipped to prevent, treat and manage any health issues that may come up. The best part? You can schedule appointments in real-time and message your provider on your secure patient portal, My Premise Health. Download the My Premise Health app or visit mypremisehealth.com to get started.

All Hearst colleagues who are enrolled in or eligible for UnitedHealthcare or Cigna medical coverage and work in the Hearst Tower or Sheffield can use primary care services offered at the Wellness Center. As always, all other employees can access acute care at the Wellness Center, including immunizations and vaccinations and treatment for common conditions such as colds, flu, infections, allergies and more, but they are not eligible for primary care services.

  • If you are enrolled in the UnitedHealthcare Consumer Plan or either of the Choice Fund Open Access Plus HSA Plans (HDHP 1600 or HDHP 3000), a primary care visit costs $35 toward your deductible. Once you reach your deductible, visits are free.
  • If you are enrolled in the UnitedHealthcare Classic Plan or the Open Access Plus Plan (OAP 600), a primary care visit costs $10 per visit.
  • If you are eligible for medical coverage through UnitedHealthcare or Cigna but waived it, primary care visits cost $50.

Please note that preventive care, such as annual physicals and screenings, is always provided at no cost to you and with no deductible when you use an in-network provider, such as the Wellness Center, and are enrolled in UnitedHealthcare or Cigna medical coverage.

Travel medicine*

At the Hearst Wellness Center, we offer travel medicine services to help you plan for your trip and to stay well while you’re away. Based on your destination, things like getting vaccinated, refilling your prescriptions and discussing health risks with your provider can make or break your travel experience.

Our travel medicine services include pre-travel risk assessment, prescriptions and vaccinations based on destination, risk reduction planning and education for self-treatment, as well as post-travel assessments and treatment, or specialist referral.

Women's health*

Women have unique healthcare needs. Our providers spend time getting to know you to best understand your health goals. They’ll come up with a personalized care plan to support your long-term health.

Along with general primary care services, we offer the following specifically for women’s health: breast cancer screenings, pregnancy testing, pelvic exams, contraception counseling, STD testing and counseling, cervical cancer screenings (Pap smears), counseling for interpersonal and domestic violence, LGBTQ+ care, comprehensive family care and education for self-awareness. Women’s health services are available for all female-identifying Hearst employees enrolled in or eligible for UnitedHealthcare or Cigna medical coverage.

Condition management*

You don’t have to navigate your health alone. With our condition management services, you can get the support you need to manage your conditions and take control of your health. Our providers will come up with personalized treatment strategies to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, diabetes and pre-diabetes, low back pain and more. We can provide one-on-one coaching, disease specific education, self-care training and condition management tactics, diet guidelines and physical activity goals, medication counseling and preventive and ongoing monitoring to manage your conditions and help you stay well.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at the Hearst Wellness Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors. You can then easily schedule an appointment or simply walk in to get your vaccines.

We offer a variety of routine vaccines to protect your health and our community, including Influenza, Hepatitis A, Hepatitis B, Human Papilloma Virus (HPV), MMR, Pneumococcal, Tdap/Td, Varicella and Zoster.

Lab services

To support your needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following lab tests onsite: glucose or lipid testing, hemoglobin A1C, rapid strep testing, influenza testing, urine pregnancy testing and urine dip screenings. We also offer COVID-19 testing through your virtual care benefits. The kit is sent to your door through our lab services partner, LabCorp, and results come back within 72 hours*. Simply request a virtual visit to get started.

Wellness

The Hearst Wellness Center takes a holistic approach to your health by focusing on your overall wellness, preventive care and chronic conditions. Our wellness programs promote nutrition and physical activity, as well as managing stress and relationships. Your wellness providers will personalize your wellness plan and help you make positive lifestyle changes for a healthier you.

Calista Yong, Certified Physician Associate

Calista Yong is a certified physician associate with a Bachelor of Science degree in Physician Assistant Studies from St. John’s University. She has been working as a physician associate for over 10 years and is passionate about educating her patients and empowering them to take control of their health. One of the most rewarding parts of her work is being able to build strong relationships with her patients, listen to their concerns and guide them through their healthcare journey. When she isn’t working, Calista enjoys exploring the world through hiking and traveling.

Jeanine Troisi, RN

Jeanine is a BSN registered nurse with over 15 years of experience in various fields of nursing, including hospice, primary care, occupational health, oncology, infectious diseases, and medical/surgical. She holds licenses in New Jersey and New York, and is CPR/BLS, PRI and SCREEN certified.

For the past 5 years, Jeanine has served as a RN field case manager in hospice for VNS Health and MJHS, collaborating with and leading an interdisciplinary team to manage patients in palliative care. She has previously worked as a triage/occupational health registered nurse at West Midtown Medical Group, primary care facility where she managed the employee health program and achieved 97% compliance with the flu shot program over two years. Jeanine has previously worked in oncology at Mount Sinai West, in primary care at Callen-Lorde Medical Center, serving the needs of the LGBT community, and on a medical/surgical unit at RWJ Barnabas Health systems in Livingston, NJ.

She has excellent leadership, critical thinking, and communication skills, and is proficient in several electronic health record systems.

She holds a Bachelor of Science in nursing degree from Drexel University, where she was inducted into the Sigma Theta Tau International Nursing Honor Society and Delta Epsilon Iota Honor Society, and a Bachelor of Arts degree in communications from William Paterson University. She is passionate about providing quality care to patients, enhancing her skills and knowledge as a nurse, and providing efficiency, support, and structure as a manager.

Frequently Asked Questions

Premise Health is a provider of in-person and virtual health for the world’s leading organizations. In other words, we partner with companies to bring their employees healthcare services and operate their wellness centers.

Members are what we call our patients who are eligible to use a wellness center operated by Premise Health. It’s not a membership that you have to pay for or apply to be a part of. You’re simply considered a member once your employer has partnered with us to provide your health and wellness services.

A wellness center is where you receive care as a member. We use this term versus the typical “doctor’s office” because we offer so much more than the usual office environment. Our centers not only typically offer more services than the average doctor’s office, but are also designed with you in mind, so you feel comfortable, calm and welcomed.

The Hearst Wellness Center is staffed, managed and operated by Premise Health. Your medical staff includes a combination of providers, nurses and medical assistants. Each staff member has been carefully screened and is qualified to deliver exceptional care. The Hearst Wellness Center is not staffed or managed by Hearst or their  employees in any way.

Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the Hearst Wellness Center.
