You have questions, we have answers. 

Premise Health is the organization that operates and staffs your health center. We’re one of the nation’s leading providers of direct healthcare, and we’re here as your health ally to help keep you at your best. We do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

You can call your center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Members enrolled in BCBSIL received preventive visits covered at 100%. Problem-focused appointments are $45, including lab work. Physical Therapy is $20 per visit. As part of your benefits, you have access to 10 free EAP visits with our Behavioral Health Counselor. Additional visits are available for $30. Care management is offered at no cost.

The Doc is staffed, managed and operated by Premise Health. The medical staff includes a combination of physicians, nurse practitioners, care management registered nurses and physical therapists. Each staff member has been carefully screened and is qualified to deliver exceptional care. Your wellness center is not staffed or managed by Devon Energy or their employees in any way.

My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.

Yes, your wellness center can perform blood draws and other lab tests ordered by an outside provider. All lab orders must be faxed or dropped off at the center to ensure we have the proper supplies for the required testing.

Preventive care includes routine checkups and screenings that help you stay healthy in the long-term. These are visits you make when you’re feeling well to catch any health issues early or improve your wellbeing. This includes annual physicals, cancer screenings, vaccinations and more.

Non-preventive care on the other hand, includes visits you schedule when you’re not feeling well. These are meant to address your symptoms to find out what’s wrong and get you back to feeling your best. Some examples include sick visits and treatment for issues like cold and flu, allergies, injuries and more.
